B细胞活化因子/增殖诱导配体在多发性骨髓瘤中的表达特点和临床意义

目的:研究B细胞活化因子(BAFF)和增殖诱导配体(APRIL)在多发性骨髓瘤(MM)患者骨髓单个核细胞(BMMCs)中mRNA表达特点及血浆中蛋白表达水平,并分析血浆中BAFF和APRIL水平与MM临床肿瘤负荷指标的相关性,初步探讨BAFF和APRIL对MM的发生、发展及预后的意义.方法:收集25例MM患者及10例正常对照的骨髓标本,获取BMMCs,并同步收取血浆.采用实时荧光定量PCR(RT-PCR)的方法检测BMMCs中BAFF和APRIL mRNA的表达,采用酶联免疫吸附试验(ELISA)法检测患者血浆中BAFF和A-PRIL的含量;使用相同方法,检测MM细胞株KM3中BAFF及APRIL的基因及蛋白表达特点.结果:①BAFF和APRIL基因和蛋白在MM组中表达升高,与对照组比较差异有统计学意义(P<0.01);②BAFF和APRIL基因和蛋白在MM初治组及未缓解组中表达升高,与缓解组比较差异有统计学意义(P<0.05),初治组与未缓解组比较差异无统计学意义;③BAFF和APRIL蛋白水平与患者外周血中ALB呈负相关(P<0.05),与外周血中β2微球蛋白(β3-MG)、免疫球蛋白(Ig)浓度及骨髓中浆细胞百分比均呈正相关(均P<0.05),MM血浆中BAFF和APRIL浓度之间呈正相关(P<0.01).结论:MM中BAFF和APRIL的基因及蛋白表达升高,MM组内初治组、未缓解组均高于缓解组,血浆中BAFF和APRIL含量与MM肿瘤负荷呈正相关.BAFF及APRIL的表达特点可能与MM的发生、发展有关,可作为评判MM预后的新指标.
