2007—2010年广西医科大学第六附属医院住院患者抗菌药物应用分析

目的:调查2007—2010年我院住院患者抗菌药物应用情况,分析抗菌药物使用的特点及其变化趋势,为临床合理用药和科学管理抗菌药物提供参考。方法:通过院内信息系统查询统计2007—2010年我院住院患者抗菌药物使用量和销售金额,采用用药金额排序法和限定日剂量(DDD)法对抗菌药物合理应用的情况进行分析与评估。结果:4年来β-内酰胺类及β-内酰胺酶抑制剂、头孢菌素类及氟喹诺酮类抗菌药物年销售金额(万元)分别占所有抗菌药物年销售金额的72.20%(1 260.53/1 745.90)、68.75%(1 429.06/2 078.67)、78.61%(2 185.85/2 780.56)和84.80%(3 061.42/3 610.48),β-内酰胺类及β-内酰胺酶抑制剂的销售金额和氟喹诺酮类药用药频度(DDDs)一直居第1位,抗真菌药用量逐年增加,排序逐年靠前。结论:我院抗菌药物用药结构比较稳定,选择和用药基本合理,但仍存在问题,应减少细菌耐药率的发生,提高临床抗感染治疗的效果,保证用药更安全、有效和经济。

Utilization Review of Antibacterials in Guangxi Medical University Affiliated Sixth Hospital During 2007-2010

OBJECTIVE: To investigate the utilization of antibacterials in our hospital during 2007-2010 period with regard to its characteristics and variation tendency so as to provide reference for clinical rational medication and scientific management of antibacterials.METHODS: The consumption data(quantity and consumption sum) of antibacterials in our hospital during 2007-2010 period collected from hospital information system and the rationality of the use of antibacterials were analyzed and evaluated using consumption sum ranking and DDD(defined daily dose) analytical method.RESULTS: Of all the antebacterials used,β-lactam and β-lactamase inhibitors,cephalosporins and fluoroquinolones accounted for 72.20%,68.71%,78.61% and 84.80% respectively in terms of annual consumption sum during 2007-2010;the consumption sum of β-lactam and β-lactamase inhibitors and the DDDs of fluoroquinolones dominated the first places steadily.The consumption quantity of antifungal drugs increased year by year and its rank has been forcing to the front year by year.CONCLUSION: The consumption structure of antibacterials in our hospital was stable,and the choice and use of which were rational on the whole but far from perfect.It is important to reduce the incidence of bacterial drug resistance and enhance curative effect of anti-infective therapy to ensure safe,effective and economical drug use.
