| 摘 要： | 目的:对1例慢性肾衰竭患者实施药学服务,达到个体化治疗的目的。方法:针对具体病例,通过检索文献为患者治疗过程中药物的使用寻找支持依据,总结治疗经验,并对患者提出合理的药学监护措施及健康教育计划。结果:临床药师为患者提供药学服务可以提高患者治疗的依从性,提高药物治疗水平。结论:临床药师对药物疗效、相互作用、药品不良反应方面具有自身优势,以患者为中心的药学服务应成为医院药学的发展方向。

| Abstract: | OBJECTIVE: To provide individualized therapy for one patient with chronic renal failure by providing the patient with pharmaceutical care.METHODS: Aimed at the patient’s state,the supporting evidence for the drug use in the treatment was searched through literature retrieval meanwhile gathering treatment experience and putting forward reasonable measures for pharmaceutical care and health education plan for the patients.RESULTS: The pharmaceutical care provided by clinical pharmacists contributed to enhancement of patients’ compliance to treatment and improvement of the drug treatment level.CONCLUSION: Clinical pharmacists have their own advantages in knowledge about drug efficacy,drug interaction and adverse drug reactions.The future pharmaceutical care should be patient-centered. |
