我院腹腔镜胆囊切除术患者围术期抗菌药物应用分析

目的:了解我院肝胆外科腹腔镜胆囊切除术患者围术期抗菌药物预防性应用情况。方法:回顾性调查我院肝胆外科2010年11月—2011年2月107例腹腔镜胆囊切除术患者的出院病历,对抗菌药物应用数据进行统计、分析。结果:患者围术期预防性应用抗菌药物达100%;抗菌药物应用率排序居前5位的分别是洛美沙星71例次(25.9%)、头孢匹罗49例次(17.9%)、头孢米诺28例次(10.2%)、奥硝唑26例次(9.5%)、头孢替安25例次(9.1%);单一用药57例(53.3%)、二联用药46例(43.0%)、三联用药4例(3.7%);初次给药在术前0.5～2 h内者3例、在48 h内停药者1例、在72 h内停药者11例,抗菌药物应用时间平均为5.1 d。结论:我院腹腔镜胆囊切除术患者围术期抗菌药物不合理应用现象较普遍,亟需引起重视。

Perioperative Use of Antibacterials in Patients Undergoing Laparoscopic Cholecystectomy

OBJECTIVE: To investigate the perioperative use of prophylactic antibacterials in patients undergoing laparoscopic cholecystectomy in our hospital.METHODS: The utilization data of antibactierials in 107 patients who underwent laparoscopic cholecystectomy from November 2010 to February 2011 were analyzed statistically.RESULTS: 100% of the patients received antibacterials;the top 5 drugs on the list of utilization frequency were lomefloxacin 71 cases(25.9%)],cefpirome 49 cases(17.9%)],cefminox 28 cases(10.2%)],ornidazole 26 cases(9.5%)] and cefotiam 25 cases(9.1%)].57 cases(53.3%) received one drug,46(43.0%) two and 4(3.7%) three in combination.3 cases received drugs within 0.5-2 hours preoperatively;discontinuation within 48 h was noted in 1 versus within 72 h in 11 patients.The mean duration for patients receiving antibacterials was 5.1 days.CONCLUSION: It is urgent to attach great importance to the widespread irrational use of antibacterials in patients undergoing laparoscopic cholecystectomy during perioperitive period in our hospital.
