清洁手术围术期预防性应用抗菌药物分析

目的:调查分析某三甲医院清洁手术围术期预防性应用抗菌药物情况,评价其合理性并提出相应的干预措施。方法:采取回顾性调查方法,抽取该院2009年1—12月出院的清洁手术病历共548份,分别填写调查表,进行统计分析。同时对外科医师进行抗菌药物认知度问卷调查,填写调查表。结果:548例手术全部使用了抗菌药物,使用率为100%。不合理用药现象主要表现为药物选择不合理、预防用药起点过高、用药时间不当、术后用药时间过长。外科医师对抗菌药物专业知识的掌握比较欠缺。结论:该院清洁手术围术期预防感染用药存在较多问题,亟待加强对外科医师规范化使用抗菌药物的管理和培训,以保证临床用药安全、有效和经济。

Analysis of Preoperative Use of Prophylactic Antibiotics in Patients Undergoing Aseptic Operation

OBJECTIVE: To investigate the perioperative use of prophylactic antibiotics in patients(from a grade-three and class-A hospital) and evaluate its rationality and put forward corresponding intervention measures.METHODS: By a retrospective study,a total of 548 surgical patients who had been discharged from this hospital during Jan.-Dec.of 2009 were enrolled to fill questionnaire with the data analyzed statistically.Meanwhile,the surgeons were asked to fill the questionnaire regarding their cognition on antibiotics.RESULTS: All of the 548 cases(100%) reviewed were administered with antibiotics.The irrationality in antibiotic use manifested as irrational choice of antibiotics,high starting point in the prophylactic drug use,improper time of administration,and prolonged postoperative drug use.The surgeons were found to have inadequate professional knowledge.CONCLUSION: There are many problems in the preoperative use of prophylactic antibiotics in patients undergoing aseptic operation.It is urgent to tighten management and training on surgeons on standardized use of antibiotics to ensure clinical safe,effective and use of antibiotics.
