对比单相抑郁与双相障碍抑郁期患者脑自发性神经元活动

摘    要:目的 采用静息态功能MRI(rs-fMRI)对比观察单相抑郁(UD)与双相障碍(BD)抑郁期患者脑自发性神经元活动(SNA)的异同。方法 纳入40例病程<24个月的UD患者(UD组)、43例BD抑郁期患者(BD组)及44名健康人(对照组),采集颅脑rs-fMRI,观察3组间及每2组间局部一致性(ReHo)存在显著差异的脑区;采用Pearson相关性分析观察上述脑区的ReHo值与基本资料的相关性。结果 3组间双侧楔前叶、左侧前扣带回及顶上小叶、右侧小脑前叶、顶上、顶下小叶及角回ReHo值存在明显差异(P均<0.05)。相比对照组,BD组右侧小脑前叶ReHo值降低(P<0.05),左侧顶上小叶及右侧顶上、顶下小叶、角回ReHo值增高(P均<0.05);UD组双侧楔前叶及右侧小脑前叶ReHo值降低(P均<0.05)。相比BD组,UD组右侧顶上、顶下小叶ReHo值降低(P均<0.05),左侧前扣带回ReHo值增高(P<0.05)。以上组间存在显著差异的脑区ReHo值与年龄、受教育年限及汉密尔顿抑郁量表-24项评分均无明显相关(P均>0.05)。结...

Abstract:Objective To observe the shared and distinct brain spontaneous neuronal activity (SNA) in patients with unipolar depression (UD) and bipolar depressive (BD) in depressive phase with resting-state functional MRI (rs-fMRI). Methods Totally 40 cases of UD (UD group) and 43 cases of BD in depressive phase (BD group) with duration of disease less than 24 months, as well as 44 healthy subjects (control group) were enrolled. Brain rs-fMRI was performed, the brain areas with significant differences of regional homogeneity (ReHo) among 3 groups and between each 2 groups were analyzed. Pearson correlation analysis was used to evaluate the correlation between ReHo values of the above brain regions and basic data. Results ReHo values of bilateral precuneus, left anterior cingulate gyrus and superior parietal lobule, right anterior lobe of cerebellum, superior and inferior parietal lobule, and angular gyrus were significantly different among 3 groups (all P<0.05). Compared with control group, ReHo values in right anterior lobe of cerebellum decreased (P<0.05) but in left superior parietal lobule, right superior and inferior parietal lobule and angular gyrus (all P<0.05) increased in BD group, while decreased in bilateral precuneus and right anterior lobe of cerebellum in UD group (all P<0.05). Compared with BD group, ReHo values in right superior and inferior parietal lobules decreased (both P<0.05) and in left anterior cingulate gyrus increased (P<0.05) in UD group. No obvious correlation was found between ReHo values of the above brain regions among groups, nor of age, years of education and Hamilton depression scale-24 scores (all P>0.05). Conclusion SNA in anterior lobe of cerebellum decreased in patients with UD and BD in depressive phase. SNA in precuneus decreased in patients with UD, in parietal lobule and angular gyrus increased in patients with BD in depressive phase.
