摘    要:目的:了解围术期肺癌患者免疫功能特点及变化规律。方法:测定并分析手术前后外周血白细胞(WBC)及淋巴细胞所占比例(L%),手术前、手术后第1天、第5天、第10天外周血1gG、NK和T淋巴细胞及其亚群CD3、CD4、CD8、CD3/DR、CD8/CD38。结果:手术后WBC与术前相仿,L%明显下降(P<0.01);手术后第10天NK上升(P<0.01),CD3、CD4、CD8均较术前下降(P<0.01),术后第5-10天恢复到术前水平;IgG术后即开始持续性下降,至术后第5、10天有非常显著意义(P<0.01)。结论:胸部手术在术后1周左右对肺癌病人免疫功能有相当大的负面作用,围术期免疫保护甚为重要。

Abstract:Objective To explore the characteristic of immunity function of patients with lung cancer in perioperative period. Methods Samples from peripheral blood of fifty patients with lung cancer before operation and at the first, fifth, tenth day after operation. WBC, L%(percentage of lymphocytes)IgG, NK, CD3, CD4, CD8, CD3 /DR, CD8 /CD38 were determined, and their relationship was evaluated statistically. Results WBC showed no marked change before and after operation. L% decreased obviously ( P <0.01) after operation. CD3, CD4, CD8 decreased obviously on the first day after operation ( P <0.01), NK increases on the frist day after operation obviously ( P <0.01). IgG decreased continously after operation till 5th and 10th day( P <0.01). Conclusion Thoracic operation has apparent negative effects on the immunity function of patients with lung cancer during the first week after operation, it is important to adopt suitable treatment for the perioperative patient.
