妊娠合并急性阑尾炎112例临床分析

目的总结妊娠期急性阑尾炎的诊治经验，防止流产与早产、死胎等并发症发生。方法回顾性分析我院2007年6月～2010年3月收治的112例妊娠合并急性阑尾炎患者的临床资料，按治疗方法不同分为两组，其中手术治疗组（78例）采用阑尾切除术治疗，保守治疗组（34例）要求保守治疗，统计观察两组的治疗效果。结果手术治疗组78例患者均手术顺利，66例经腹单纯阑尾切除术，10例剖宫产加阑尾切除术，2例术后引产；术后无切口感染，均Ⅰ期愈合，无产妇死亡。保守治疗组34例患者中28例治疗无效改行手术治疗；6例保守治疗患者症状缓解，无流产、早产等并发症。结论妊娠合并急性阑尾炎早期明确诊断、尽早手术是提高治愈率，降低并发症的关键。

Clinical Analysis of 112 Cases of Pregnancy Complicated Acute Appendicitis

Objective To summarize the experience of acute appendicitis during pregnant period to prevent the complication of abortion premature and fetal death, etc. Methods Retrospectively reviewed 112 cases admitted by our hospital from June 2007 to March 2010 of acute appendicitis in pregnant women and observed the curative effect of the two groups. 112 cases were divided into two groups. 78 cases were operated,34 cases were conservatinely treated. Results 78 cases had successful surgery including 66 patients of the abdominal simplex appendectomy, 10 cases with cesarean section before appendectomy,2 got induced labour after operation, no incision infection or death occurred. 28 cases with conservative treatment turned to surgery, and 6 cases were in relief, in which no abortion and early delivery occurred. Conclusion The key to operate in time is the main approach for acute appendicitis during pregnancy period to increase recovery rate and decrease the rate of complications.
