听神经瘤术中面肌肌电图的应用分析

目的研究听神经瘤术中面肌肌电图监护对面神经保留的作用。方法对28例听神经瘤经枕下入路行肿瘤全切除术，术中进行面肌肌电图实时监护。结果全部患者均全切肿瘤，面神经解剖保留率为92．9％，术后1个月和半年H—B分级Ⅰ～Ⅱ级为82．1％，其中术末面神经刺激强度为0．1～0．5mA，波幅〉1．0mV。结论在听神经瘤术中行面肌肌电图监护可以有效提高面神经保留率。

Application of Intraoperative Electromyography Monioring During Acoustic Neuroma Surgery

Objective To study the effect of intraoperative electromyography（EMG） monitoring on protection of the facial nerve during acoustic neuroma surgery. Methods The tumors were removed completely by the way of sub-occipital approaches in 28 patientes with acoustic neuromas and the facial electomyography monitoring was clone during operation. Results All of the tumors were totally resected,92.9% facial nerve were anatomically preserved. According to H0use-Brackmann facial nerve grading system,82.1% of them were grade I ~ 1~ at 1 month and 6 months postoperation,of which stimulation threshold of the facial nerve at the end of the operation range from 0.1 to 0.5mA and response amplitude of 1.0mV or greater. Conclusion During operation on acoustic neuroma, the intraoperative electromyography monitoring could increase the function preservation rate of the facial nerve.
