降钙素基因相关肽在出血性脑卒中的变化

目的动态观察出血性脑卒中患者血清及脑脊液中降钙素基因相关肽（CGRP）的变化，探讨二者之间的关系及其对预后的影响。方法采用EHSA法检测40例患者发病1、3．7、14d的血清和脑脊液中的CGRP水平，结合患者入院时的GCS评分及发病第30天的GOS评分进行数据统计分析。结果患者血液和脑脊液中CGRP水平在发病后即有明显升高，3d达高峰，7d后逐渐下降；在发病初的1～7d，GCS评分越低血清及脑脊液中的CGRP值越高（P〈0．001），预后不良组CGRP水平明显高于预后良好组，两者有统计学差异（P〈0．05）；同一患者脑脊液CGRP值较血液含量高（P〈0．05）。结论患者血液和脑脊液中CGRP水平变化与病情严重程度有关，血液和脑脊液CGRP含量呈正相关（r=0．746）。CGRP作为一种神经多肽，其水平变化能反映出血性脑卒中后神经损伤的严重程度，而且可为出血性脑卒中的预后评估提供参考依据。

The Change of Calcitonin Gene Related Peptide in the Hemorrhagic Stroke

Objective To dynamic observation of changes of calcitonin gene related peptide（CGRP） in blood and cerebrospinal fluid （CSF）of patients with hemorrhagic stroke in acute stage and the relationship between them. Methods Statistical analysis of data that 40 cases of hemorrhagic stroke patients in 1st day,3th days,7th days andl4th days of serum and cerebrospinal fluid levels of CGRP were detected by enzyme-linked immunosorbent assay（ELISA） in combination with the GCS score on admission of patients and the incidence of the first 30 days of the GOS score and other information. Results The CGRP levels in the serum and cerebrospinal fluid reached maximum at 3th day and decreased after 7th day. In the early stage after onset （without 14th days） at the corresponding time points,the lower GCS score,the higher CGRP values（P〈0.001）. The CGRP levels of the worse prognosis group visible higher than the excellent prognosis group and there were deferences between them（P〈0.05）.In the same patient,the value of CGRP in CSF was higher compared to the blood ones （P〈0.05）. Conclusion The levels of CGRP in hemorrhagic stroke were associated with the severity of the disease. CGRP levels in blood and cerebrospinal fluid was positively correlated （r=-0.746）. CGRP is an neuropeptide what can reflect the level of brain stroke and the severity of neurologic injury,but also provide,a reference of hemorrhagic stroke prognosis.
