摘    要:目的探讨Hamm和Carroll公式计算酸碱平衡紊乱预计代偿范围的差别。方法将739份实测异常血气分析结果同时用Hamm和Carroll公式判断酸碱失衡类型,研究两组公式的一致性。结果两组公式判断代谢性酸中毒一致率为83.78%(Kappa值0.67,P〈0.01),判断代谢性碱中毒一致率为54.17%(Kappa值0.21,P〈0.01),判断急性呼吸性酸中毒一致率为81.36%(Kappa值0.53,P〈0.01),判断急性呼吸性碱中毒一致率为43.93%(Kappa值0.15,P〈0.05),判断慢性呼吸性酸中毒一致率为78.08%(Kappa值0.56,P〈0.01),判断慢性呼吸性碱中毒一致率为54.72%(Kappa值0.25,P〈0.01)。结论用Hamm和Carroll公式判断酸碱失衡类型存在明显差别。

Abstract:Objective To compare the difference between Hamm and Carroll formulae for expected acid-base compensation in acid-base imbalance.Methods Arterial blood gas analysis results of acid-base disorder from 739 subjects were evaluated respectively by Hamm and Carroll formulae,and the concordance was judged.Results For metabolic acidosis,the concordance rate was 83.78% between Hamm and Carroll (Kappa value =0.67,P0.01),for metabolic alkalosis,the concordance rate was 54.17%(Kappa value =0.21,P0.01),for acute respiratory acidosis,the concordance rate was 81.36% (Kappa value =0.53,P0.01),for acute respiratory alkalosis,the concordance rate was 43.93% (Kappa value =0.15,P0.05),for chronic respiratory acidosis,the concordance rate was 78.08%( Kappa value =0.56,P0.01) and for chronic respiratory alkalosis,the concordance rate was 54.72% (Kappa value=0.25,P0.01).Conclusion The difference between Hamm and Carroll formulae for acid-base imbalance is obvious.
