改良细小导管与普通气管导管在喉显微外科手术中的应用比较

目的探讨使用不同气管导管对喉显微外科手术的影响和安全性。方法选择支撑喉镜下喉显微外科手术60例,随机均分为普通气管导管组和改良细小导管组,在全身麻醉时普通气管导管组经口腔插入普通气管导管,改良细小导管组经鼻腔插入改良细小导管。记录两组术野的暴露满意度,连续监测并记录3个不同时点两组的心率（HR）、收缩压（SBP）、舒张压（DBP）和脉搏血氧饱和度（SpO2）。结果两组术中麻醉效果均满意,对应各时点的HR、SBP、DBP、SpO2比较,差异无统计学意义（P〉0.05）;术野暴露满意度改良细小导管组优于普通气管导管组,差异也具有统计学意义（P〈0.05）。结论改良细小导管应用于支撑喉镜下喉显微外科手术,术野暴露满意度好,安全、可靠,并发症少。

A Comparison of Application between Reform Small Airway and General Endotracheal Tube in Microlaryngoscopic Surgery

Objective To compare the influence and safety of different endotracheal tubes in microlaryngoscopic surgery. Methods 60 patients scheduled for microlaryngoscopic surgery were randomly divided into greneral endotracheal tube group and reform small airway group, with 30 cases in each. HR, SBP, DBP and SpO2 were monitored and the degree of satisfaction was recorded to field of view. Results There was no significant difference in HR, SBP, DBP, SpO2 before and after treatment between the two groups（P 〉 0. 05）. But there was significant difference in the degree of satisfaction to field of view between the two groups（P 〈0. 05）. Conclusion It is safe and convenient to use reform small airway in microlaryngoseopic surgery, and it has a clearer field of view and less complications.
