89 例小细胞肺癌不同化疗方案疗效分析

目的：比较目前常用的 Ｃ Ｅ、 Ｅ Ｐ 化疗方案治疗小细胞肺癌的疗效。方法：１９９６ 年１ 月～１９９８ 年１０ 月应用小细胞肺癌病人８９ 例，应用 Ｃ Ｅ、 Ｅ Ｐ、 Ｔ Ｐ 方案治疗观察疗效和不良反应。结果：局限期３７ 例（４１６ ％ ） ，广泛期５２ 例（５８４ ％ ） 。初治１９ 例（６６３ ％ ） ，复治３０ 例（３３７ ％ ） 。化疗２ 周以上者作疗效评价，８９ 例中有８４ 例可评价。全组总有效率（ Ｃ Ｒ ＋ Ｐ Ｒ） ７６２ ％ ， 其中局限期、广泛期病例分别为８９５ ％ 、６５２ ％ （ Ｐ＜ ００１） ，初、复治病例分别为８３１ ％ 、６００ ％ （ Ｐ＜ ００２５） 。 Ｃ Ｅ、 Ｅ Ｐ、 Ｔ Ｐ 各组疗效分别为７０７ ％ 、７９５ ％ 、７５ ％ （ Ｐ＞ ０１） 。结论：各组化疗方案对小细胞肺癌均有较高的缓解率，特别是局限期或初治病例，但前两组方案对广泛期或复治病例疗效较差。 Ｔａｘｏｌ 联合 Ｄ Ｄ Ｐ 治疗小细胞肺癌疗效较高。

Study of chemotherapy for 89 cases with small cell lung cancer

Objective:To explore the short term efficacy of the routine regimens (CE,EP) and TP(Taxol and DDP)regimen for small cell lung cancer.Methods:From Jan. 1996 to Oct.1998,89 patients of small cell lung cancer were treated with CE,EP,TP regimens. The short efficacy and the side effects were observed.Results:All patients contain 37 limited-disease cases(LD 41.6%) and 52 extensive-disease cases (ED 58.4%).59 patients were previously untreated(66.3%) and 30 patients were previously treated (33.7%).The objective response was observed in 76.2% of patients while 89.5% of cases in LD arms versus 65.2% of cases in ED arms( P <0.01).83.1% of previously untreated patients and 60.0% of previously treated patients responsed to chemotherapy ( P <0.025).Conclusions:All regimens were effective to small cell lung cancer, especially to the LD patients and previously untreated patients. The response rate of CE,EP regimens was poor to patients with ED or the second line treatment.The efficacy of TP regimen was noticeable to small cell lung cancer.
