锥颅减压抢救急性硬膜下血肿并脑疝形成21例观察

目的　探讨锥颅减压术在抢救硬膜下血肿并脑疝的治疗效果。方法　在控制颅压 ,应用脱水剂的基础上 ,紧急施锥颅术引流血肿和脑脊液 ,消除血肿。结果　锥颅引流后 ,患侧瞳孔缩小 ,光反射恢复正常 ,昏迷程度变浅。 2 1例中死亡 4例 ,植物生存 2例 ,重残 5例 ,中残 7例 ,良好 3例。结论　急性硬膜下血肿并脑疝 ,死亡率极高 ,实施锥颅减压能有效地进行抢救

Observation on Acute Epidural ematoma combined wit Hernia Treated wit Sull Drilling

Objective To discuss the clinical effect of skull drilling to treat epidural hematoma combined with hernia. Methods On the base of reducing intracranial pressure and mannitol, the emergency skull drilling was performed to drainage spinal fluid and remove hematoma. Results After drilling, the pupil constricted and light reflex recovered without deep coma. 4 of 21 cases died and 2 cases were vegetable survival, 5 cases were severe disability, 7 cases were moderate disability and 3 were good. Conclusion The skull drilling could be effective method to treat epidural hematoma combined with hernia.
