酮胺氧化酶法测定糖化白蛋白的评价

摘    要:目的对酮胺氧化酶法测定糖化白蛋白(GA)进行方法学评价。方法分析及评价Lucica GA-L试剂盒的精密度、稀释线性、回收率、干扰因素以及试剂稳定性。比较GA与糖化血红蛋白(HbA1c)的相关性,GA与果糖胺(GSP,NBT法)的相关性。比较溴甲酚绿法(BCG)和溴甲酚紫法(BCP)测定白蛋白的相关性。结果该试剂精密度高,批内CV〈1.4%,批间CV〈2.4%;稀释线性良好;GA的平均回收率为96.74%,ALB的平均回收率为96.73%;葡萄糖、乳糜、胆红素对酶法检测GA无明显干扰,但是血红蛋白浓度在1.82 g/L以上时,对结果的影响超过10%。试剂可稳定4周以上。GA与HbA1c相关性良好(r=0.8940,P〈0.001)。BCG法测定白蛋白的值(-x±s,37.1±8.4 g/L)较BCP法(-x±s,33.3±10.0 g/L)高,二者相关性良好(r=0.9908,P〈0.001)。当ALB〉30 g/L时,GA与GSP相关良好(r=0.8474,P〈0.001);当ALB〈30 g/L时,GA与GSP相关性差(r=0.5697,P〈0.05)。结论该方法分析性能良好,能定量分析糖化白蛋白,并计算其与白蛋白的比值,受到的影响因素较少,能较好的监测短期内平均血糖水平。

Abstract:Objective To evaluate Ketoamine oxidase method of determinating glycated albumin(GA).Methods To evaluate Lucica GA-L kit by studying the imprecision,dilution linearity,recovery,interference factors,and reagent stability.Analyze the correlation between GA and glycated hemoglobin(HbA1c);GA and fructosamine(GSP,NBT method).Compare bromocresol green method(BCG) with bromocresol purple method(BCP) of albumin determination.The results showed good precision,within-run CV 1.4%,between-run CV 2.4%;good linearity;the average recovery rate of 96.74% for GA,and the average recovery rate of 96.73% for ALB.Glucose,chyle and bilirubin had no significant interference on GA,but the hemoglobin affected the results of over 10% when the concentration was over 1.82 g/L.Results was stable for more than 4 weeks.It showed excellent correlation(r=0.894 0,P0.001) between GA and HbA1c.Determination of albumin values with BCG method(±s,37.1 ±8.4 g/L) were higher than that of BCP method(±s,33.3±10.0 g/L),but with excellent correlation(r=0.990 8,P0.001).When ALB concentration was over 30 g/L,GA was well correlated to GSP(r=0.847 4,P0.001);When ALB concentration was less than 30 g/L,they were in poor correlation(r=0.569 7,P0.05).Conclusion This method of GA determination shows good performance,and it can calculate the ratio of GA and albumin.There are fewer interference factors affecting this method.This method is a good index to monitor short-term average blood glucose levels.
