摘    要:目的:探讨剪切波弹性成像(SWE)定量参数E_(max)与乳腺癌免疫组化指标CK5/6、p63、AR、EGFR的相关性。方法:术前对166例乳腺癌结节行常规超声和SWE检查。常规超声获取病灶最大径,SWE检查测量病灶最大弹性值E_(max)。以术后病理结果为金标准,记录病灶组织学分级、有无淋巴结转移以及免疫组化标志物CK5/6、p63、AR和EGFR。单因素分析采用Spearman秩相关,将单因素分析有统计学差异的变量纳入多元线性回归。结果:单因素分析结果显示,E_(max)与病灶大小、组织学分级、有无淋巴结转移中的差异有统计学意义(P<0.05),E_(max)在p63、AR阳性表达组与阴性表达组间有统计学差异(P<0.05)。多元线性回归分析结果显示,仅病灶大小与Emax呈显著相关(P<0.05)。结论:SWE定量参数E_(max)与乳腺癌病灶大小、组织学分级、腋窝淋巴结转移及p63、AR和EGFR的表达有关。术前SWE检查可能成为预测乳腺癌患者预后的一种新技术手段。

Abstract:Objective:To investigate the correlation between the quantitative parameter Emax of shear wave elastography(SWE) and the immunohistochemical indexes of breast cancer,such as CK5/6,p63,AR and EGFR.Methods:Conventional ultrasound and SWE examination were performed in 166 cases of breast cancer nodules before operation.The maximum diameter of the lesion was obtained by conventional ultrasound,and the maximum elastic value Emax was measured by SWE.According to the postoperative pathological results as the gold standard,the histological grade of the lesion,lymph node metastasis and immunohistochemical markers CK5/6,p63,AR and EGFR were recorded.Spearman rank correlation was used in univariate analysis,and the variables with statistical differences in univariate analysis were included in multiple linear regression.Results:Univariate analysis showed that there were significant differences between Emax and lesion size,histological grade and lymph node metastasis(P<0.05).There was significant difference in),Emax between p63 and AR positive expression group and negative expression group(P<0.05).Multiple linear regression analysis showed that only the lesion size was significantly correlated with Emax.Conclusion:The quantitative parameter Emax of SWE was related to tumor size,histological grade,axillary lymph node metastasis and the expression of p63,AR and EGFR.Preoperative SWE examination may be a new technical method to predict the prognosis of patients with breast cancer.
