免疫增强型肠外营养对胃癌根治术患者营养状况及免疫功能的影响

摘    要:目的:探究免疫增强型肠外营养对胃癌根治术患者营养状况及免疫功能的影响。方法:选取2018年01月至2019年12月80例在本院行胃癌根治术的患者为研究对象,根据术后给予肠外营养剂的不同,随机分为试验组和对照组,每组各40例。对照组患者给予常规肠外营养液治疗,试验组患者给予免疫增强型肠外营养液治疗。观察两组患者营养治疗前后营养状况指标、炎症因子水平及免疫功能指标的变化情况。结果:治疗后,两组患者血清前白蛋白(prealbumin,PAB)、白蛋白(albumin,ALB)、转铁蛋白(transferrin,TRF)水平均比治疗前升高,体重、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平均比治疗前降低(P<0.05);且治疗后试验组患者血清PAB、TRF、ALB水平及体重明显高于对照组,IL-6、TNF-α水平明显低于对照组(P<0.05)。治疗后,两组患者外周血T淋巴细胞亚群CD4+细胞占比、CD4+/CD8+细胞比率及免疫球蛋白A(immunoglobulin A,IgA)、免疫球蛋白G(immunoglobulin G,IgG)、免疫球蛋白M(immunoglobulin M,IgM)水平均比治疗前升高,CD8+细胞占比降低(P<0.05);且治疗后试验组患者CD4+细胞占比、CD4+/CD8+细胞比率及IgA、IgG、IgM水平均明显高于对照组,CD8+细胞占比明显低于对照组(P<0.05)。结论:胃癌根治术后患者应用免疫增强型肠外营养治疗效果好,可有效改善患者营养状况,降低机体炎症反应,提高患者免疫功能。

Abstract:Objective:To investigate the effects of immunoenhanced parenteral nutrition on the nutritional status and immune function of patients undergoing radical gastrectomy.Methods:A total of 80 patients who underwent radical gastrectomy in our hospital from January 2018 to December 2019 were selected as the study objects,and according to the different parenteral nutrition,they were randomly divided into the experimental group and the control group,with 40 cases in each group.Patients in the control group were treated with conventional parenteral nutrition solution,while patients in the experimental group were treated with immunoenhanced parenteral nutrition solution.The changes of nutritional status,inflammatory factors levels and immune function were observed before and after nutrition treatment.Results:After treatment,the levels of serum prealbumin (PAB),albumin (ALB),transferrin (TRF) in the two groups were higher than those before treatment,while the weight,the levels of interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) were lower than those before treatment (P<0.05).After treatment,the levels of serum PAB,TRF,ALB and weight in the experimental group were significantly higher than those in the control group,and the levels of IL-6 and TNF-α were significantly lower than those in the control group (P<0.05).After treatment,the percentage of CD4+ cells,the ratio of CD4+/CD8+ cells and the levels of immunoglobulin A (IgA),immunoglobulin G (IgG) and immunoglobulin M (IgM) in the two groups were higher than those before treatment,and the percentage of CD8+ cells decreased (P<0.05).After treatment,the percentage of CD4+ cells,the ratio of CD4+/CD8+ cells and the levels of IgA,IgG and IgM in the experimental group were significantly higher than those in the control group,and the percentage of CD8+ cells was significantly lower than that of the control group (P<0.05).Conclusion:The application of immunoenhanced parenteral nutrition in patients with gastric cancer after radical operation has a good effect,which can effectively improve the nutritional status of patients,reduce the inflammatory response of the body,and improve the immune function of patients.
