胰腺癌组织中Ki67、p53的表达及其与高剂量少分次放疗生存时间的关系

摘    要:目的:分析Ki67、p53在胰腺癌组织中的表达水平,及其与患者接受高剂量少分次放疗生存时间的关系。方法:回顾性收集55例胰腺癌患者接受高剂量少分次放疗前的活检病理组织,采用免疫组化SP法检测Ki67及p53在胰腺癌组织中的表达水平,并采用Log-rank检验比较不同表达水平组间生存时间的差异,Cox模型行多因素分析统计。结果:Ki67、p53蛋白在穿刺组织中的表达率分别为41.8%、47.3%,单因素生存分析显示Ki67、p53阴性表达组的生存时间在统计学上明显长于阳性组(P均<0.05),多因素分析提示Ki67、p53及远处转移均是胰腺癌患者接受高剂量少分次放疗生存时间的独立预后影响因素。结论:放疗前活检病理组织中Ki67、p53蛋白表达水平是预测胰腺癌高剂量少分次放疗生存时间的重要指标。

Abstract:Objective:To analyze the expression of Ki67 and p53 in pancreatic cancer tissues and their relationship with the survival time of high-dose hypofractionated radiotherapy.Methods:The biopsies of 55 patients with pancreatic cancer were collected retrospectively before receiving high-dose hypofractionated radiotherapy.Immunohistochemical SP method was used to detect the expression of Ki67 and p53 in pancreatic cancer.Log-rank test was used to compare the difference of survival time between different expression levels.Cox model was analyzed by multivariate analysis.Results:The expression rates of Ki67 and p53 protein in puncture tissues were 41.8% and 47.3%,respectively.Univariate survival analysis showed that the survival time of Ki67 and p53 negative expression group was significantly longer than that of positive group (P<0.05).Multivariate analysis showed that Ki67,p53 and distant metastasis were independent prognostic factors of pancreatic cancer patients receiving high-dose hypofractionated radiotherapy.Conclusion:The expression level of Ki67 and p53 protein in pathological tissue before treatment is an important index to predict the survival time of high-dose hypofractionated radiotherapy for pancreatic cancer.
