罗格列酮和二甲双胍对2型糖尿病患者胰岛素抵抗和颈动脉内膜中层厚度的影响

摘    要:目的本研究旨在分析2型糖尿病(T2DM)患者胰岛素抵抗(IR)、空腹胰岛素(FINs)与颈动脉内膜中层厚度(CIMT)相关性,并探讨应用药物罗格列酮和二甲双胍干预治疗后观察对上述指标的影响,进而探讨两种药物联合对心血管潜在的保护作用。方法选取初发2型糖尿病患者141例,然后随机分为3组,一组给予罗格列酮(RSG组),另一组给予二甲双胍(Met组),第3组给予罗格列酮和二甲双胍(联合组),进行为期12周的用药观察,并比较治疗前后指标。结果治疗12周后3组患者空腹胰岛素水平(FINs)、胰岛素抵抗指数(IRI):混合组优于RSG组(P<0.05)和Met组(P=0.000)。结论两种胰岛素增敏剂都可降低2型糖尿病胰岛素抵抗(IR)、空腹胰岛素(FINs),两药联合应用效果更为明显,能延缓2型糖尿病患者动脉粥样硬化进展,但对颈动脉内膜中层厚度短期时间内影响较小。

Abstract:Objective To analyze the relationship between insulin resistance,FINc and carotid intima-media thickness in type 2 diabetes mellitus,and to observe the therapetic effects of rosiglitazone and metformin,and investigate the potential cardiovascular protective effect of thiazolidinediones and biguanides and the early atherogenic risk factors in patients with type 2 diabetes mellitus.Methods 141 subjects with type 2 diabetes mellitus were selected and divided randomly into three groups: rosiglitazone,metformin and alliance groups,then the three groups were given different medicine.For 12 weeks,then compared the changes of above indexes with treatment.Results After treatment FINs and IRI of alliance group were showed more a decrease than rosiglitazone group(P<0.05) and metformin group(P<0.000).three groups before and after treatment the level of CIMT were not showed a significantly decrease(P>0.05).Conclusion Resiglitazone and metformin may reduce more fasting insulin and insulin resistance than dependent each one,but three medicine have no influence on CIMT in short time.
