超微血管成像技术鉴别诊断乳腺肿瘤

摘    要:目的 探讨超微血管成像(SMI)技术对乳腺肿物的鉴别诊断价值。方法 对82例资料完整的乳腺肿瘤患者(91个病灶)进行二维灰阶、CDFI检查,用传统指标构建乳腺影像报告及数据系统(BI-RADS)分类评估,并进行SMI检查,比较SMI与CDFI血管检出的差异。结合SMI成像特点对BI-RADS分类重新评估,分析SMI在BI-RADS评估中的价值。结果 CDFI血流检出率78.02%(71/91),SMI血流检出率85.71%(78/91)。SMI模式下良恶性肿物Adler分级差异无统计学意义(P=0.06),中心血流与穿支血管有助于定性诊断。SMI结合BI-RADS分类诊断乳腺恶性肿物的敏感度为95.45%(42/44),特异度为89.36%(42/47),准确率为92.31%(84/91)。结论 在传统超声表现基础上,SMI有助于对乳腺肿物定性诊断,对BI-RADS分类调整更适用于富血供肿物。

Abstract:Objective To explore the clinical value of superb microvascular imaging (SMI) in differential diagnosis of breast masses. Methods Totally 82 patients (91 lesions) with complete data were examined using 2D gray-scale, CDFI and breast imaging reporting and data system (BI-RADS). SMI were also performed and comparing the difference between CDFI and SMI in detecting vessels of breast masses. The role of SMI in BI-RADS was analyzed. Results Detection rate of blood flow with CDFI was 78.02% (71/91), and with SMI was 85.71% (78/91). Different Adler grades in SMI had no statistically significant difference (P=0.06). Central blood flow and penetrating vessels were helpful for qualitative diagnosis. Sensitivity, specificity and accuracy in SMI combination with BI-RADS classification in diagnosis of malignant breast tumor were 95.45% (42/44), 89.36% (42/47) and 92.31% (84/91). Conclusion On the basis of traditional ultrasonic manifestations, SMI is conducive to differential diagnosis of breast masses, and the adjustment of BI-RADS is more suitable for masses with abundant blood supply.
