社区卫生服务的实践与模式探讨

本文介绍了华南师范大学校医院自2000年开展社区卫生服务以来,大力开展六位一体功能,在社区疾病诊治、慢性病管理、健康教育、传染病管理等工作上的一些主要做法和经验,并率先在高校医院开设托老康复中心,方便了社区居民的养老。在开展社区卫生服务的过程中,发现最大的难点是对慢性病的管理。文章结合实际工作体会,对社区卫生服务模式进行了探讨。

Explore the Mode and Practice of Community-Based Health Service

The hospital of South China Normal University has achieved a lot in community-based health service in community diagnosis,management of chronic disease,healthy education and management of infection disease et al,since it began the community based health service from 2000.Our center carried out center for the aged in university for the first time,which takes good care of older community people.The management of chronic disease is the most difficult which existed in service now.
