甘精胰岛素在2型糖尿病胃肠道手术患者的血糖控制的疗效观察

目的探讨甘精胰岛素在2型糖尿病胃肠道手术患者的血糖控制的疗效。方法选择38例接受外科胃肠道手术的2型糖尿病患者,分为甘精胰岛素治疗组(LAN组)19例,胰岛素泵皮下持续输注胰岛素治疗组(CSII组)19例。观察两组患者治疗后空腹、三餐后2h、睡前及凌晨2时血糖变化,比较两组一天胰岛素用量和治疗期间低血糖发生率。结果各组治疗后各时间点血糖均控制均较理想,LAN组与CSII组比较,差异均无统计学意义(禁食期间,t分别=1.74、1.36、1.82、1.47,P均>0.05;进食期间,t分别=1.43、0.87、0.99、1.57、1.73、1.24,P均>0.05);LAN组低血糖发生例数与CSII比较,差异也无统计学意义(χ2=0.63,P>0.05)。结论甘精胰岛素在糖尿病胃肠道手术患者术后降糖效果显著,能迅速、平稳控制血糖且患者依从性好,是一种安全、有效且患者易接受的药物。

Effect of insulin glargine on glycemic control in type 2 diabetic patients after gastrointestinal tract operation

Objective To investigate the effect of insulin glargine in type 2 diabetic patients after gastrointestinal tract operation. Methods A total of 38 type 2 diabetic patients after gastrointestinal tract operation were randomly divided into two groups： Lantus （insulin glargine, LAN） （LAN group, n=19）, continuous subcutaneous insulin infusion group （CSII group, n：19）. Level of blood sugar, dose of insulin used and incidences of hypoglycemia were observed. Results The levels of blood sugar at all points were significantly lower after treatment, but there was no statistically significant difference between the two groups, nor the incidences of hypoglycemia （X^2=0.63 ,P〉0.05）.While during absolute diet, t= 1.74,1.36,1.82,1.47,P〉0.05; after taking food, t=1.43,0.87,0.99,1.57,1.73,1.24, P〉0.05. Conclusions Glargine is effective on stably blood sugar control in type 2 diabetic patients after gastrointestinal tract operation, and the patients have good compliance.
