广西抗结核组合药间歇疗法中断治疗原因及不良反应调查

目的调查和分析广西涂阳肺结核病患者使用抗结核药间歇疗法中断治疗原因及不良反应的主要表现和发生率,以提出相应的处理措施。方法对2004年和2005年上半年接受国家提供的免费抗结核药物治疗的初治涂阳肺结核病患者20464例及复治涂阳肺结核病患者6593例中断治疗的发生率和原因以及抗结核药物各种不良反应率进行回顾性分析。结果初治和复治涂阳肺结核病患者因各种原因中断治疗率分别为9.47%和17.57%,其中因服用抗结核药物出现不良反应而中断治疗占所有中断治疗原因的首位,初治和复治涂阳肺结核病患者相似,接近29%。27057例涂阳肺结核病患者中,有6756例患者出现不同程度、表现不一的不良反应,不良反应发生率为24.97%,并有892例患者因此而中断治疗,因不良反应而中断治疗率为11.45%。结论不良反应是广西涂阳肺结核病患者中断治疗的主要原因,应引起相关部门重视,并采取相应的措施以降低因不良反应而引起的中断治疗,确保抗结核治疗方案的顺利完成。

Reasons of treatment interruption and side-effects of anti-tuberculotic pharmacotherapy in Guangxi Zhuang Autonomous Region

Objective To investigate the reasons of treatment interruption and side-effects of free anti-tuberculotic pharmacotherapy in Guangxi Zhuang Autonomous Region.Methods Relevant data of 20464 newly diagnosed and 6593 retreated sputum smear positive tuberculosis patients who received free anti-tuberculotic medicines in 2004 and the first half of 2005 in Guangxi were collected and analyzed.Results The rates of treatment interruption because of different reasons in the newly diagnosed and retreated sputum smear positive patients were 9.47% and 17.57% respectively.The reasons of treatment interruption included side effect(28.80%),death of the patients(21.54%),failure of treatment(17.99%),becoming migrant workers(7.43%),refusal to treatment(5.78%),changing the dwelling place(5.30%),economic reason(2.81),complication(2.20%),etc.Failure to tolerate the side-effects was the number one reason of treatment interruption.6756 patients suffered from side-effects of different degrees and symptoms and signs,with an incidence of 24.97%,and the treatment of 892 out of the 6756 patients was interrupted consequently with a treatment interruption rate of 11.45%.Conclusion The main reason of treatment interruption in the sputum smear positive patients in Guangxi is side-effect.Effective measures should be taken to alleviate such side reaction so as to successful accomplish the anti-tuberculotic treatment.
