大剂量碘油抗癌药乳剂同轴微导管法栓塞肝癌瘤体的临床应用研究

摘    要:目的 改进栓塞肝癌的方法 ,减少并发症 ,提高治疗效果和远期生存率。方法 碘油分别与 5 Fu、MMC混合乳剂 ,采用同轴微导管法 ,大剂量序列栓塞肝癌瘤体 160例 ,观察效果及术后反应并与相关文献资料对照。结果 化疗栓塞后临床治愈率 2 0 % ,显效率 5 5 % ,有效率 17.5 % ,无效或微效 7.5 % ,效果高于文献报道。栓塞术后综合征发生率为 86.2 % ,略低于文献资料·。结论 碘油与两种化疗药分别混合栓塞可延长化疗药与癌灶接触时间 ,提高两药协同性抗癌效果。大剂量碘油有利于混合大剂量化疗药 ,加重栓塞程度 ,延长碘油流失时间 ,促进肝癌细胞的迅速坏死 ,减少肝内转移的机会。同轴微导管法的应用 ,避免了正常肝血管、细胞遭受LP化疗药的损害 ,使大剂量碘油化疗药乳剂栓塞更具有安全和高效性。

Abstract:Objective To improve the embolization approaches for hepatic carcinoma and to reduce the complications caused by embolization.Methods 160 patients with hepatic carcinoma were treated with high dose lipiodol sodium mixed with 5 Fu and MMC and with the coaxal microtube.The efficiency and reaction were observed and compared with the relative literatures.Results After having been embolized,32 patients with hepatic carcinoma were cured clinically,88 patients showed prominent therapeutic efects,28 patients effetive,and 12 patients showed no effect.Conclusion The embolization with lipiodol sodium mixed with 5 FU and MMC could maintain the medicine in the masses for longer time and improve synergentic effect on anticarcinoma. The coaxal microtube can prevent 5 Fu and MMC from damaging the normal hepatic cell and make the therapy safe and high effective with high dose lipidol sodium mixed with 5 FU and MMC. FU
