髋关节后脱位并髋臼骨折21例手术治疗的临床效果

目的观察髋关节后脱位并髋臼骨折手术治疗的临床效果。方法对21例创伤性髋关节后脱位并髋臼骨折的患者按照髋关节后脱位Thompson-Epstein分型,Ⅱ型:5例,Ⅲ型:12例,Ⅳ型:3例,Ⅴ型:1例,施行手术切开整复骨折,髋臼重建钢板内固定术治疗。结果术后随访1~4年,平均2.25年。按Merled,Aubigne髋关节评分系统评分标准评定:优18例,良2例,可1例,差0例。优良率95.2%。结论采用手术切开整复骨折,髋臼重建钢板内固定术治疗髋关节后脱位并髋臼骨折效果良好。

Clinical effects of surgery for treatment of 21 patients with posterior dislocation of hip joint and acetabular fracture

Objective To investigate clinical effects of surgery for the treatment of posterior dislocation of hip joint associated with acetabular fracture.Methods Twenty-one cases of traumatic posterior dislocation of hip joint associated acetabular fracture were typed according to Thompson-Epstein on posterior dislocation of hip joint, typeⅡ was 5 cases ,type Ⅲ was 12 cases,type Ⅳ was 3 cases ,type Ⅴ was 1 case.All patients were treated with incision and reduction of bone ,and acetabular reconstruction internal fixation with steel plate.Results All of 21 cases were followed up for 1～4 years(mean 2.25 years).Assessed by Merle d,Aubigne acetabular scoring system,18 cases were rated as excellent,2 case as good,1 case as fair.The excellent and good rate was 95.2%.Conclusion Application of incision and reduction and acetabular reconstruction internal fixation with steal plate in the treatment of posterior dislocation of hip joint associated with acetabular fracture is effective.
