颈前路减压植骨内固定治疗下颈椎骨折

摘    要:目的探讨前路减压、钉板系统固定结合自体髂骨或钛网植骨治疗下颈椎骨折的疗效。方法应用颈前路减压、钉板系统固定结合自体髂骨或钛网植骨治疗32例患者。C31例、C45例、C512例、C65例、C4骨折并脱位2例、C5骨折并脱位5例、C6骨折并脱位2例。结果除4例完全截瘫患者外,其余病例神经功能有不同程度恢复。术后随访钢板位置良好,无植骨块塌陷或脱出。结论颈前路减压、钉板系统固定结合植骨治疗下颈椎骨,可减少并发症的发生,防止脊髓进一步损伤,促进神经功能恢复。

Abstract:Objective To investigate the curative effects of anterior decompression nail plate system combined with autologous ilium or titanium grafted bone for lower cervical spine fractures. Methods Cervical anterior decompression combined with autologous ilium or titanium grafted bone were performed on 32 patients. 1 case of C3, 5 cases of C4, 12 cases of C5, 5 cases of C6, 2 cases of C4 fracture dislocation, 2 cases of C6 fracture dislocation, 22 cases of Reflex nail plate, 10 cases of Orion nail plate. Results In all these cases, neurological function have recovered with different extent except for 4 cases who were paralytics. Follow-ups were performed, it was found that the position of steel plate was well located, no grafted bones were dented or prolapsed. Conclusions Cervical anterior decompression nail plate system combined with bone grafting can reduce complications, prevent spinal marrow from further injury, and accelerate recovery of neurological function.
