格林-巴利综合征95例临床治疗分析

目的研究成人格林-巴利综合征（GBS）的临床特点，更进一步探讨其治疗方法及预后。方法对我院收治的95例GBS例患者的临床资料进行回顾性分析，并将所有患者分成激素治疗组、免疫球蛋白治疗组、联合应用激素及免疫球蛋白治疗组，比较三组患者的治疗效果。结果本病临床表现复杂。激素治疗组轻型治愈率50．0％，中型治愈率25．0％，重型治愈率10．0％，极重型治愈率0。免疫球蛋白治疗组轻型治愈率100％，中型治愈率44．4％，重型治愈率36．4％，极重型治愈率22．2％。联合应用激素和免疫球蛋白组轻型治愈率100％，中型治愈率62．5％，重型治愈率62．5％，极重型治愈率42．9％。2例患者死于呼吸机麻痹及重症肺部感染，2例患者死于心脏骤停。结论格林-巴利综合征病因复杂，临床表现复杂多样，电生理及脑脊液检查对诊断有特殊意义。激素对于轻、中型患者治疗有一定疗效，免疫球蛋白尤其适用于重及极重型患者。激素与免疫球蛋白联合应用疗效优于单用。GBS大多数预后良好。

Clinical therapy analysis of 78 patients with GBS

Objective To study the clinical features of adult Green - Barre syndrome （GBS）and further discuss the treatment and prognosis. Method We performed retrospective analysis of the original data of the 95 GBS patients of our hospital. All patients were divided into hormone therapy group, immunoglobulin therapy group and combination group of hormone and immune globulin, compared three groups＇s treatment result. Result The disease＇s clinical features were complex. The cure rates of hormone therapy group from light type to extremely serious type were respectively 50.0% ,25.0% ,10.0% and 0. The cure rates of im- munoglobulin therapy group from light type to extremely serious type were respectively 100%, 44.4%, 36.4%, 22.2%. The cure rates of combined group from light type to extremely serious type were respectively 100%, 62.5%, 62.5% and 42.9%.Two patients died of respiratory paralysis and severe local lung infection, and another two cases died of cardiac arrest. Conclusion The causes and clinical features of GBS were complex.Electrophysiological and cerebrospinal fluid examination had a special significance in the diagnosis of GBS. To light and medium type patients, the treatment of hormone had a positive effect. Immunoglobulin was particularly suitable to serious and extremely serious types.The combination of the two were more effective than single-use. Most patients of GBS had a good prognosis.
