人生回顾访谈法对胰腺癌患者心理状况和希望水平的影响

目的探讨人生回顾访谈法对姑息治疗胰腺癌患者的希望水平和心理状况的影响，为晚期癌症患者的临终服务提供理论依据。方法选择在湖南省怀化市第三人民医院普外科住院并接受姑息治疗的胰腺癌患者71例，随机分为干预组（36例）和对照组（35例），干预组给予人生回顾干预措施，对照组仅给予常规护理，干预时间1个月，应用综合医院焦虑抑郁量表和希望水平量表评估患者的干预效果。结果干预组胰腺癌患者干预后焦虑得分为（7．89±2．31）分，对照组为（9．22±2．53）分，差异有统计学意义（t=-2．126，P〈0．05），抑郁情绪干预组比对照组明显降低（P〈0．05）；干预后干预组总希望水平有所提高，与对照组比较差异有统计学意义（38．60±4．57）比（34．73±4．89）分，t=3．167，P〈0．01]，各维度差异均有统计学意义（P〈0．05或P〈0．01）。结论人生回顾访谈法能较好的改善患者的心理状况，提高其对生活的希望水平，从而提高患者生命最后阶段的生活质量。

Effect of life review interviews on mental health and hoping level of pancreatic cancer patients

Objective To explore effect of life review interviews on mental health and hoping level of pancreatic cancer patients, and to provide theoretical basis for deathbed service for patients.Methods 71 pancreatic cancer patients were chosen by convenient sampling and were randomly divided into the experimental group and control group.The experimental group adopted life review interviews intervention; the control group was only given the general nursing guidance.The experiment time was 1 month, the effects were evaluated by respectively before and after the intervention with hospital anxiety and depression scales and hope index.Results The anxiety level were decreased in the experimental group (7.89±2.31) vs (9.22 ±2.53), t =2.126,P＜0.05], as well as depression level (P＜0.05).Hoping level in experimental group were obviously improved (38.60 ±4.57) vs (34.73 ±4.89),t =3.167, P ＜0.01]; there was statistically significant differences of all the dimensions between the two groups (P ＜0.05 or P ＜ 0.01).Conclusions The life review interviews had a positive effect in improving mental health and hoping level of pancreatic cancer patients.
