血清肌钙蛋白Ⅰ水平与不稳定型心绞痛预后的关系

摘    要:目的 探讨心脏肌钙蛋白Ⅰ (cTnⅠ )对不稳定型心绞痛 (UA)的预后价值。方法 对 48例UA患者、19例SA患者和 30例正常人 ,采用化学发光法分别测定cTnⅠ、肌酸激素酶同功酶 (CK MB)、肌红蛋白 (Mb)的浓度 ,依据cTnⅠ≥ 0 .1μg/L或 <0 1μg/L ,将患者分为cTnⅠ升高组和cTnⅠ正常组 ,观察其住院期间心脏事件发生率。结果  48例UA患者中 ,2 8例 (5 8 3% )cTnⅠ≥ 0 1μg/L ,2 0例 (4 1 7% )cTnⅠ <0 1μg/L ,分别为 (0 15 1±0 0 90 ) μg/L和 (0 0 79± 0 0 2 0 ) μg/L(P <0 0 1) ;住院期间cTnⅠ升高组发生心脏事件的频率为 5 0 0 % ,明显高于cTnⅠ正常组的 2 0 0 % (P <0 0 5 )。结论 cTnⅠ是反映心肌损伤的敏感、特异性生化指标 ,有助于UA的治疗干预和预后判断。

Abstract:Objective To study the prognostic significance of cardiac troponin I(cTnI) in patients with unstable angina pectoris(UA). Methods Serum cTnI, creatine kinase-MB(CK-MB) and myoglobin (Mb) were chemiluminescencely measured in 48 cases with UA and 19 with stable angina pectoris(SA) and 30 others as control. Based on a formula of cTnI≥0.1g/L or<0.1g/L, the patients with UA were accordingly divided into cTnI elevated group and cTnI normal group whose frequency of cardiac events in hospital were observed. Results Serum cTnI level in 28 patients with UA was found to be 0.1g/L or more (58.3%) and less than 0.1 g/L in 20 patients(41.6%)(0.151±0.090 vs 0.079±0.020, P<0.01). There was high percentage of cardiac events in cTnI elevated group(46.4% vs. 20.0%, P<0.01). Conclusion cTnI, a special indicator of myocardial injury, contributes to the treatment and prognosis of UA patients.
