胆道镜在腹腔镜胆道探查术中的应用价值(附334例报告)

目的探讨胆道镜在腹腔镜胆管探查术中的应用价值。方法应用术中胆道镜配合腹腔镜胆道探查术治疗复杂性胆道结石334例。结果344例中取净结石323例；11例中转开腹，其中术后残余结石6例，无其它严重并发症。术后住院时间明显缩短，平均手术时间也短于开腹手术。217例随访半年以上未见远期并发症。结论胆道镜的应用使腹腔镜胆道探查术更安全、更能体现出其微创手术的价值。

Value of choledochoscopy for laparoscopic bile duct exploration: a report of 334 cases

Objective To determine the value of choledochoscopy for laparoscopic bile duct exploration. Methods Choledochoscopic cholelithotomy and laparoscopic choledocholithotomy were performed to treat complicated common bile duct stones in 334 patients. Results The operation was successfully performed in 323 patients and was converted to open procedure in 11. Six of the former 323 patients had residual stones. No severe complication was found in all the patients. The average duration of laparoscopic choledocholithotomy was shorter than that of open procedure. The duration of hospitalization was shortened. The follow-up in 217 patients for 6 months showed that there was no postoperative complication. Conclusions The use of choledochoscope in laparoscopic bile duct exploration is practicable.
