脐血乳酸与新生儿窒息及行为神经评分相关分析

目的:探讨脐血乳酸值(LD)与新生儿窒息及窒息后新生儿行为神经功能(NBNA)测定的相关因素,预测新生儿预后。方法:阴道和剖宫产无窘迫无窒息组310例为对照组,有窘迫无窒息为Ⅰ组,有窒息为Ⅱ组,分别测定新生儿脐血乳酸和行为神经功能(NBNA)测定。结果:新生儿窒息组脐血乳酸水平明显高于对照组及Ⅰ组,三组比较P<0.01,有显著意义。NBNA评分对照组与Ⅰ组比较P>0.05,无显著意义,对照组与Ⅱ组比较P<0.01,有显著意义。结论:新生儿脐血乳酸水平与胎儿窘迫、新生儿窒息的发生呈直线正相关,脐血乳酸水平与新生儿行为神经功能NBNA评分在胎儿窘迫无窒息组无直线相关,在新生儿窒息组呈直线负相关。

The Correlation between Umbilical Artery Lactate Levels and Neonatal Asphyxiation,and NBNA Scores

Objective:To discuss the correlation between umbilical artery lactate levels and neonatal asphyxiation,and correlated factors of neonate,Neonatal Behavioral Neurological. Assessment( NBNA ),in order to predict the prognosis of the newhom infants .Methods:Choose the neonates in the vaginal delivery and the cesarean section and divide them into three groups, the neonates of no distress and no asphyxiation as control group,the neonates with distress but no asphyxiation as group I,and the asphyxial neonates as group II.Measure theumbilical artery lactate levels and the NBNA of the three groups.Results:Umbilical artery lactate levels of group II were significant higher than the other two groups(P<0.01).There was no significant difference of NBNA scores between the control group and group I (P>0.05).However there were significant difference between the control group and the group II (P<0.05).Conclusion:There is a positive correlation between the neonatal lactate levels and fetal distress,neonatal asphyxiation,and there is a negative correlation between the neonatal lactate levels and the neonatal scores of NBNA in asphyxiation group,however there is no correlation in distress group.
