大肠腺瘤异型增生及癌变的潜在危险因素分析

目的探讨大肠腺瘤发生异型增生及癌变的潜在危险因素。方法对发生大肠腺瘤异型增生及癌变的腺瘤大小、解剖部位、外形、病理组织学类型及患者的年龄、性别等因素进行回顾性分析。结果686例大肠腺瘤患者中发生癌变者36例,癌变率为5·3%。年龄≥45岁者与<45岁者大肠腺瘤异型增生程度间差异有显著性意义(P<0·05);不同直径、外形、病理类型的大肠腺瘤的异型增生程度及癌变率间差异有显著性意义(P<0·01);不同部位大肠腺瘤异型增生程度间差异有显著性意义(P<0·01)。结论腺瘤的大小、解剖部位、外形、病理组织学类型及患者年龄是大肠腺瘤异型增生及癌变的潜在危险因素。

Study on Potential Factors of Heterotypic Dysplasia and Cancerization of Colorectal Adenoma

Objective To study the potential factors of heterotypic dysplasia and cancerization of colorectal adenoma.Methods The size,location,appearance and pathological type of colorectal adenoma as well as the age and sex of patients were retrospectively analyzed.Results In all the 686 cases,the rate of cancerization was 5.25% (36 cases).The dysplasia the patients above 45 years old had a higher rate than those less than 45 years old(P<0.05).The rate of cancerization and dysplasia of tubular villous adenoma had significant difference in defferent size,appearance and pathlolgical type(P<0.01).The rate of dysplasia had significantly difference in different location of colorectal adenoma.Conclusion The degree of heterotypic dysplasia and cancerization of colorectal adenoma are closely related to its size,location,appearance and pathological type,as well as the age of patients,but not to the sex of patients.
