股骨头置换和内固定治疗老年股骨颈骨折疗效比较

目的比较股骨头置换术和内固定术治疗老年股骨颈骨折的临床疗效。方法2003年1月至2008年12月湖南省祁阳县人民医院分别采用股骨头置换术和空心钉内固定术治疗60岁以上老年有移位的股骨颈骨折67例。平均随访24．2个月，从手术时间、术中出血量、术后负重、术后再手术率、Harris髋关节功能评价等方面进行回顾性比较分析。结果股骨头置换组与内固定组之间各项指标比较差异有统计学意义（P〈0．05）。结论股骨头置换术治疗老年股骨颈骨折优于内固定术。

Comparision on femoral replacement and internal fixation of senile femoral neck fracture

Objective To compare the femoral replacement and internal fixation of femoral neck fractures in senile clinical efficacy. Methods From January 2003 to December 2008, femoral replacement surgery and cannulated screw fixation were performed in our hospital received the treatment on 60 or over 60 years pa- tiens with displaced femoral neck fractures of 67 cases. The average follow-up was 24. 2 months. Comparative a- nalysis of the operative time, blood loss, postoperative weight-bearing, reoperation rate, Harris hip function e- valuation was done. Results The results of internal fixation of femoral replacement group and the difference between groups of indicators were significant （ P 〈 0. 05 ）. Conclusion Treatment of senile femoral replacement is better than internal fixation of femoral neck fracture.
