145例转移性肝癌血管造影影像分析

目的：通过对１４５ 例转移性肝癌的血管造影影像分析，以求提高对转移性肝癌与原发性肝癌的鉴别诊断水平，并初步判断原发癌部位。方法：回顾分析１４５ 例转移性肝癌ＤＳＡ表现，其中原发癌明确的１３４ 例，１１例行ＤＳＡ后诊断为肝转移癌。结果：转移性肝癌与原发性肝癌在ＤＳＡ影像上有明显区别：１．无明显供血动脉；２．实质期肿瘤染色；３．无门脉瘤栓；４．无Ａ—Ｖ瘘；５．可见门脉供血。结论：掌握肝转移癌的ＤＳＡ影像特征可及时确定肝内病变性质，指导临床治疗，对临近脏器恶性肿瘤尚可判断其来源。

The Analysis of DSA Manifestations of Metastatic Tumor of the Liver in 145 Cases

Objective:To differentiate metastatic hepatic tumor from primary hepatic tumor,and to find the primary site,the DSA manifestations of metastatic hepatic tumor in 145 cases were analysed.Methods:DSA manifestations of metastatic hepatic tumor in 145 cases were studied retrospectively,of them,134 cases were the primary carcinoma and 111 cases were diagnosed as metastatic carcinomas after IADSA.Results:The metastatic hepatic carcinoma is differential obviously from the primary hepatic carcinoma as follow:1.There was no supply artery obviously;2.The stains of tumors in solidity phase;3.There was no portal cancerous embolus;4.There was no A-P fistula;5.Supply vessel from portal vein could be seen.Conclusion:To master the DSA features of metastatic hepatic carcinoma can soon help to determine the nature of hepatic tumor and to guide the therapy of interventional of radiology.The primary carcinoma can be discovered by DSA if the primary site is neighboring.
