以深呼吸为节拍引导足踝主动屈伸运动对下肢静脉回流的影响

摘    要:目的探讨以深吸气引导足踝背伸30°、深呼气引导足踝跖屈45°的主动屈伸运动对下肢静脉回流的影响,以寻找简便易行的预防下肢深静脉血栓形成的方法。方法2012年1-6月便利抽样法选取30名健康受试者,采用自身前后对照的方法,比较单纯足踝主动屈伸运动与以深吸气引导足踝背伸30°、深呼气引导足踝趾屈45°运动时股静脉的血流动力学指标及受试者的主观评价。结果以深呼吸为节拍引导足踝主动屈伸运动与单纯足踝主动屈伸运动相比较,前者股静脉血流峰速度、血流速度增大时的持续时间及血流量均大于后者,差异均有统计学意义(P〈0.05);且受试者对前者的依从性及掌握程度均优于后者,差异均有统计学意义(P〈0.05或P〈0.01)。结论以深呼吸为节拍引导足踝主动屈伸运动能更有效地促进下肢静脉血液回流,有效降低静脉血栓形成的风险;且该方法简单易行,便于掌握。

Influence of Ankle Initiative Flexion and Extension Movement Guided with Deep Breath as Beat on Lower Limb Venous Return

Abstract:Objective To investigate the influence of initiative flexion and extension movement of deep-in- spiration-guided ankle dorsiflexion for 30° and deep-exhalation-guided flexor for 45° on lower limb venous return,and to find out the simplest and most feasible method for preventing the formation of deep vein thrombosis. Methods From January to June 2012,the hemodynamics indexes and the subjective evaluation were conducted on the femoral vein of 30 healthy adults under the movement state of simple ankle initiative flexion and extension movement, deep-breathing-guided ankle dorsiflexion for 30° and deep-breathing- guided flexor for 45° using before and after self-control method. Results Compared with simple ankle initi- ative flexion and extension movement, the effect of the ankle initiative flexion and extension movement guided with deep breath as beat was more significant for increasing femoral vein blood flow peak velocity (P〈0.05) ;the duration when the blood flow velocity increases was longer (P〈0.05) ;the blood flow was also significantly greater (P〈0.05);the compliance and mastery level were significantly improved(P〈0.05 or P〈0.01). Conclusion Ankle initiative flexion and extension movement guided with deep breath as the beat can more effectively promote venous blood return, reduce the incidence of deep vein thrombosis. And also the method is simple to grasp.
