针刺从脾论治高脂血症的临床研究

目的：观察从脾论治取穴针刺对高脂血症脂类代谢和血清一氧化氮(NO)的影响，探讨该法调整脂类代谢的可能机制。方法：将45例患者随机分为3组(均嘱予低脂饮食)：药物组、针刺组、对照组，观察各组治疗前、后血脂[总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL—C)、低密度脂蛋白胆固醇(LDL—C)]、致动脉粥样硬化指数[AI：脂蛋白(sp0)B／ap0AI]及NO的变化。结果：除对照组外，在调整脂类代谢方面，其他两组治疗后较治疗前均有不同程度的改善(P＜0．05)，组间比较，差异无显著性意义(P＞0．05)；在改善AI方面，其他两组治疗后均较治疗前有不同程度的降低(P＜0．01)，组间比较，针刺组较药物组效果好(P＜0．05)；对血清NO水平的影响，治疗后其他两组在原低水平上有不同程度的升高(P＜0．01)，组间比较，针刺组较药物组效果好(P＜0．05)。结论：从脾论治取穴针刺调整脂类代谢疗效确切，可作为防治高脂血症及动脉粥样硬化的重要辅助手段。其作用机制可能与提高高脂血症患者血清NO水平、降低AI有关。

Clinical Effect of Acupuncture on Patients with Hyperlipemia in Theory of Spleen

Objective: To observe the clinical effect of acupuncture on patients with hyperlipemia in theory of spleen and investigate the mechanism of regulating the lipid metabolism and preventing and treating atherosclerosis (AS) by acupuncture. Methods: Forty-five patients with hyperlipemia were divided into three groups at random (all were asked to take low lipid food) ; drug group, acupuncture group and control group. The elbow venous blood of all patients was collected to examine the level of blood lipid (TC, TG, HDL, LDL) and NO before and after treatment. The AI was calculated accordingly. Results: Except control group, all subjects in other groups demonstrated significant improvement in blood lipid, AI, NO ( P <0. 05 or <0. 01). Conclusion: Acupuncture in theory of spleen had reliable effect of regulating the metabolism of lipid and might be taken as an important measure to prevent and treat hyperlipemia and AS. Its regulatory mechanism may be relevant to increasing the level of blood NO of hyperlipemia patients so as to prevent the progress of AS.
