电针联合肌电生物反馈疗法对痉挛性偏瘫患者H波及M波的影响

摘    要:目的:探讨电针联合肌电生物反馈疗法对痉挛性偏瘫患者脊髓运动神经元兴奋性的影响。方法:选取脑卒中后痉挛性偏瘫患者44例,将其随机分为对照组及治疗组,分别采用肌电生物反馈训练及电针联合肌电生物反馈疗法进行治疗,观察两组患者治疗前后肌电图H波、M波最大波幅及H波/M波最大波幅比值的变化情况,作为判定脊髓运动神经元兴奋性变化的客观指标。结果:两组患者经2个疗程(28d)治疗后H/Mmax均降低,且治疗组H/Mmax降低幅度明显优于对照组(P<0.01)。结论:经电针联合肌电生物反馈训练治疗后,对脑中风后遗症患者脊髓运动神经元异常活动的抑制效果明显好于单纯肌电生物反馈训练疗法,其机制可能是通过交互抑制的反射活动,对脊髓α运动神经元兴奋性产生了调整作用,使痉挛侧肌群肌紧张度下降,从而缓解肌痉挛状态。
