核素显像法观察放射治疗对鼻咽癌患者腮腺功能的影响

摘    要:目的利用核素显像法观察放疗对鼻咽癌患者腮腺功能的影响。方法常规根治放疗患者4例分别于放疗前、放疗疗程结束第1天、放疗后3个月行99mTcO4-腮腺动态显像;6例常规放疗及11例适形调强放疗的鼻咽癌患者于放疗前、放疗后显像,结合影像学表现和半定量指标Ex值对腮腺功能进行判断。结果4例行常规放疗的患者放疗后腮腺功能明显下降(P<0.05);放疗结束第1天与3个月腮腺功能无显著性差异(P>0.05);常规放疗与适形调强放疗后Ex值下降平均数分别为41.10%和21.11%(P<0.01),两组均未出现极度损伤。结论鼻咽癌患者常规根治放疗后其腮腺功能受损,3个月后未见明显恢复;适形调强放疗法较常规放疗法对腮腺的损伤较小;腮腺动态显像是一种合适的监测方法。

Abstract:Objective To study the effect of radiotherapy on parotid function of patients with nasopharygeal carcinoma (NPC) by nuclide imaging. Methods 4 cases treated with common radiotherapy (CRT) were examined by 99mTcO4- parotid dynamic imaging prior to radiotherapy, first day and three months after radiotherapy; 6 cases treated by CRT and 11 cases treated by intensity modulated radiotherapy (IMRT) were examined by 99mTcO4- parotid dynamic imaging prior and at the end of radiotherapy. Parotid function was estimated acording to which combined with image manifestation and semi-quantiwive index the maximum excretion rate (Ex) value. Rusults Parotid function of 4 cases treated by CRT decreased significautly after radiotherapy (P<0. 05) but no sinificautly difference in paraticl function was founed between the results of first day and three months after radiotherapy (P>0. 05). Average Ex value decreased 41. 10% and 21. 11% aspectively after CRT and IMRT (P <0. 01). Extreme damage was not found in two groups. Conclusions Parotid function of NPC patients will be damaged after CRT, no apparent recovery appeared after three months. The damage to parotid of IMRT is slighter than that of CRT. Parotid dynamic imaging is a suitable monitoring method.
