低功率CO_2激光焊接鼠小肠的实验研究

摘    要:目的探讨低功率CO2激光焊接小肠的可行性。方法将60只SD大鼠分为缝合组和激光组,每组各30只。分别以功率0.15 W的CO2激光焊接或临床外科常规缝合鼠小肠,比较两组吻合的时间、术后死亡率和术后第14天各肠管吻合口粘连、狭窄程度、破裂压及组织病理学变化。结果激光组和缝合组的吻合时间分别为(25+6.24)min和(36+7.50)min,差异有显著意义(P〈0.05)。根据Sertz's评分标准分级激光组和缝合组粘连均值分别为26.12和34.88,差异有显著意义(P〈0.05)。术后第14天两组狭窄指数接近,差异无显著意义(P〉0.05)。破裂压测定:激光组破裂压比缝合组高,且破裂口多位于吻合口以外的正常肠管。激光组吻合口周围炎症反应轻,管壁光滑;缝合组可见明显的炎症反应,管壁不光滑。结论低功率CO2激光焊接小肠组织是可行的,具有一定潜在的临床应用价值。

Abstract:Objective To investigate the feasibility of intestinal anastomsis in rats with low-power carbon CO2 laser.Methods Sixty SD rats were divided into suture group and laser group,30 rats in each group.The intsetine anastomosis in laser group was done by using 0.15W output power CO2 laser and compared with traditional stitch suturing.The welding time,mortality,adhesion,constriction,bursting pressure and histological findings in each group were compared 2 weeks after operation.ResultsAnastomosis time of the laser group was 25±6.24 minutes and that of the suture group was 36±7.50 minutes.There was a difference between the two groups(P0.05).According to Sertz's adhesion scoring,the mesn adhesion value was 26.12 and 34.88 in laser and suture groups respectively(P0.05).Stenosis index of the two groups was similar 14 days after operation(P0.05).Burst pressure of laser group was higher than that of suture group and most ruptures were located outside the anastomosis.Pathological changes showed there were light foreign body reaction and smooth wall in laser group but no significant foreign body reaction and smooth.ConclusionsLaser anastomosis is a new and quite efficient technique.
