宫颈环形电切术治疗宫颈高级别鳞状上皮内病变的疗效及预后影响

目的 观察宫颈环形电切术（LEEP）治疗宫颈高级别鳞状上皮内病变（HSIL）的疗效及预后影响。方法 选取60例HSIL患者，根据手术方法不同分为LEEP组与冷刀锥切术（CKC）组，每组30例。LEEP组患者实施宫颈环形电切术治疗，CKC组患者实施冷刀锥切术治疗。比较两组围手术期指标、术后人乳头瘤病毒（HPV）转阴率、宫颈成型率及妊娠率。结果 LEEP组患者术中出血量明显少于CKC组，手术时间、住院时间均短于CKC组，差异有统计学意义（P<0.05）；两组宫颈切除面积比较，差异无统计学意义（P>0.05）。LEEP组术后1个月宫颈成型率96.67%明显高于CKC组的76.67%，差异有统计学意义（P<0.05）。随访6～12个月，LEEP组术后HPV转阴率为90.00%,CKC组术后HPV转阴率为93.33%，比较差异无统计学意义（P>0.05）;LEEP组妊娠率56.67%明显高于CKC组的16.67%，差异有统计学意义（P<0.05）。结论 LEEP在HSIL治疗中，可明显减少术中出血量，缩短手术时间及住院时间，提高术后HPV转阴率、宫颈成型率及妊娠率，...

The curative effect and prognostic effect of cervical ring electrotomy in the treatment of High-grade squamous intraepithelial lesions of the cervix

Objective To observe the curative effect and prognostic effect of circular electric excision of cervix(LEEP)in the treatment of High-grade squamous intraepithelial lesion(HSIL).Methods 60 patients with HSIL were divided into LEEP group and Cold knife conical resection(CKC)group according to different surgical methods,with 30 patients in each group.Patients in LEEP group were treated with annular electric cervical resection,and patients in CKC group were treated with cold knife conical resection.The perioperative indicators,postoperative human papillomavirus(HPV)negative conversion rate,cervical formation rate and pregnancy rate were compared between two groups.Results The amount of intraoperative blood loss in LEEP group was significantly less than that in CKC group,and the operation time and hospital stay were shorter than those in CKC group,with statistical significance(P<0.05).There was no significant difference in cervical resection area between the two groups(P>0.05).The cervical molding rate of LEEP group was 96.67% significantly higher than that of CKC group(76.67%),the difference was statistically significant(P<0.05).After 6~12 months of follow-up,the negative HPV conversion rate was 90.00% in the LEEP group and 93.33% in the CKC group,with no statistical significance(P>0.05).The pregnancy rate of 56.67%in LEEP group was significantly higher than that in CKC group(16.67%),the difference was statistically significant(P<0.05).Conclusion In the treatment of HSIL,LEEP can significantly reduce intraoperative blood loss,shorten the operation time and hospital stay,and improve the negative HPV conversion rate,cervical fbnnation rate and pregnancy rate,with ideal effect.
