纤维支气管镜灌洗治疗小儿重症肺炎的临床疗效探析

摘    要:目的 分析有创呼吸机辅助通气下纤维支气管镜灌洗治疗小儿重症肺炎的临床疗效及对氧疗时间的影响。方法 在医院2019年3月—2021年2月接收的重症肺炎患儿中抽选50例作为观察对象,应用简单随机分组法将其分为对照组和观察组,每组各占25例。对照组患儿给予有创呼吸机辅助通气治疗,观察组患儿接受有创呼吸机辅助通气下纤维支气管镜灌洗治疗。比较两组患儿的情况。结果 观察组患儿治疗后的血小板比容(platelet hematocrit,PCT)、C反应蛋白(C-reactive protein,CRP)、动脉血氧分压(PaO2)、氧合指数(PaO2/FiO2)、动脉血二氧化碳分压(PaCO2)、咳嗽缓解时间、肺啰音消失时间以及体温恢复时间、呼吸机使用时间、总氧疗时间、住院时间均明显优于对照组,差异有统计学意义(P <0.05)。结论 有创呼吸机辅助通气下纤维支气管镜灌洗治疗小儿重症肺炎的临床疗效显著。

Abstract:Objective To analyze the clinical efficacy of fiberoptic bronchoscopy lavage under invasive ventilator assisted ventilation in the treatment of severe pneumonia in children and its influence on the duration of oxygen therapy.Methods A total of fifty children with severe pneumonia admitted to our hospital from March 2019 to February 2021 were selected as observation subjects,and were divided into control group and observation group by simple random grouping method,with 25 cases in each group.The control group was received invasive ventilator assisted ventilation,while the observation group was received fiberoptic bronchoscopy lavage under invasive ventilator assisted ventilation.Compared the two groups of children.Results The observation group after treatment in children with platelet hematocrit(PCT)and C-reactive protein(CRP),arterial blood oxygen partial pressure(PaO2),oxygenation index(PaO2/FiO2),arterial blood CO2partial pressure(PaCO2),cough reduce time disappear,lung sounds and temperature recovery time,breathing machine use time,total oxygen cure time,length of hospital stay were significantly superior to the control group,the difference was statistically significant(P<0.05).Conclusion Fiberoptic bronchoscopy lavage under invasive ventilator assisted ventilation is effective in the treatment of severe pneumonia in children.
