Application value of combined detection of CYFRA21-1,CA153,SCC and CRP in the diagnosis of lung cancer

Abstract:Objective To explore the application value of combined detection of cytokeratin 19 soluble fragment(CYFRA21-1),carbohydrate antigen 153(CA153),squamous cell carcinoma antigen(SCC) and C-reactive protein(CRP) in the diagnosis of lung cancer.Methods From January 2018 to December 2019,150 patients with solitary pulmonary nodules suspected of lung cancer treated in our hospital were included in the study,and the levels of CYFRA21-1,CA153,SCC and CRP were measured.The diagnostic sensitivity,specificity,accuracy,positive predictive value and negative predictive value of single and combined detection CYFRA21-1,CA153,SCC and CRP for lung cancer were compared;the levels of serum CYFRA21-1,CA153,SCC and CRP were compared between lung cancer group and benign lung lesions group;the levels of serum CYFRA21-1,CA153,SCC and CRP in lung cancer patients with different pathological types,clinical stages,lymph node metastasis and non-lymph node metastasis were compared;the correlation between the levels of serum CYFRA21-1,CA153,SCC,CRP and the pathological classification,clinical stage and lymph node metastasis of lung cancer patients were analyzed.Results Among the 150 patients with solitary pulmonary nodules,110 were diagnosed as lung cancer by pathological diagnosis of puncture biopsy,and the other 40 cases were benign pulmonary nodules.According to the pathological diagnosis results of puncture biopsy,the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of the combined detection in the diagnosis of lung cancer were higher than those of the single detection(P<0.05).The levels of serum CYFRA21-1,CA153,SCC and CRP in the lung cancer group were higher than those in the benign pulmonary nodules group(P<0.05).Among 110 patients with lung cancer,there were 39 patients with small cell lung cancer and 71 patients with non-small cell lung cancer;clinical stage:27 cases in stage Ⅰ,35 cases in stage Ⅱ,28 cases in stage Ⅲ and 20 cases in stage Ⅳ;lymph node metastasis occurred in 35 cases and no lymph node metastasis in 75 cases.The levels of serum CYFRA21-1,CA153,SCC and CRP in patients with small cell lung cancer were higher than those in patients with non-small cell lung cancer(P<0.05).The levels of serum CYFRA21-1,CA153,SCC and CRP in lung cancer patients with stage Ⅰ,Ⅱ,Ⅲ and Ⅳ were significantly different(P <0.05).The levels of serum CYFRA21-1,CA153,SCC and CRP in patients with lymph node metastasis were higher than those in patients with non-lymph node metastasis(P<0.05).The correlation analysis results showed that the levels of serum CYFRA21-1,CA153,SCC and CRP were positively correlated with pathological classification,clinical stage and lymph node metastasis of patients with lung cancer(P<0.05).Conclusion The combined detection of CYFRA21-1,CA153,SCC and CRP has good application value in the diagnosis of lung cancer,and can assist in the differential diagnosis of pathological classification and clinical stage of lung cancer.
