产前超声征象评分在前置胎盘合并胎盘植入中的诊断价值

目的 研究产前超声征象评分在前置胎盘合并胎盘植入中的诊断价值。方法 纳入2016年1月至2020年12月我院接收的110例前置胎盘孕妇展开研究，按照临床及病理检查结果将其分为未植入组（62例）和植入组（48例）。同时纳入30例正常孕妇作为正常组。比较未植入组与植入组的产前超声征象和不良妊娠结局；比较三组的产前超声征象评分，同时对植入组中胎盘植入不同类型的产前超声征象评分进行观察；分析产前超声征象评分与临床和病理检查结果的一致性及产前超声征象评分的诊断效能。结果 植入组的子宫肌层最薄处<2 mm、胎盘增厚、胎盘后间隙部分或完全消失、宫颈膨大检出率均高于未植入组，产后出血、子宫全切或次全切除、早产、新生儿窒息发生率均高于未植入组，差异具有统计学意义（P<0.05）。植入组的产前超声征象评分高于未植入组、正常组，差异具有统计学意义（P<0.05）。产前超声征象评分对前置胎盘合并胎盘植入的真阳性检出例数为46例，与临床和病理检查结果的一致性较好（k=0.875,P<0.05）。产前超声征象评分的灵敏度、特异度、准确度、阳性预测值、阴性预测值分别为95.83%、93.55...

Diagnostic value of prenatal ultrasound sign score in placenta previa complicated with placenta accreta

Objective To study the diagnostic value of prenatal ultrasound sign score in placenta previa complicated with placenta accreta.Methods A total of 110 pregnant women with placenta previa received in our hospital from January 2016 to December 2020 were included in the study.According to the clinical and pathological examination results,they were divided into non-implantation group(62 cases)and implantation group(48 cases).At the same time,30 normal pregnant women were included as normal group.The prenatal ultrasound sign and adverse pregnancy outcomes between the non-implantation group and the implantation group were compared;the prenatal ultrasound sign score of the three groups was compared,and the prenatal ultrasound sign score of different types of placental accreta in the implantation group were observed;the consistency of prenatal ultrasound sign score with clinical and pathological examination results and the diagnostic efficacy of prenatal ultrasound sign score were analyzed.Results The detection rates of the thinnest part of uterine myometrium<2 mm,placental thickening,partial or complete disappearance of posterior placental space and cervical dilatation in the implantation group were higher than those in the non-implantation group,and the incidences of postpartum hemorrhage,total or subtotal hysterectomy,preterm birth and neonatal asphyxia were higher than those in the non-implantation group,the differences were statistically significant(P<0.05).The prenatal ultrasound sign score of the implantation group was higher than that of the non-implantation group and the normal group,and the difference was statistically significant(P<0.05).The number of true positive cases of placenta previa complicated with placenta accreta in prenatal ultrasound score was 46,which was in good consistency with the clinical and pathological examination results(k=0.875,P<0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of prenatal ultrasound sign score were 95.83%,93.55%,94.55%,92.00%and 96.67%respectively.Conclusion Prenatal ultrasound sign score has a high diagnostic efficiency in the diagnosis of placenta previa complicated with placenta accreta and can predict the prognosis.
