Clinical effect of massive costal cartilage graft in rhinoplasty to correct nasal base depression

Objective To compare the advantages and disadvantages and long-term effects of the two methods through the clinical observation of the effect of mass cost cartilage grafts and granular cost cartilage grafts on the correction of nasal base depression in patients with prosthetic costal cartilage rhinoplasty.Methods A total of 249 patients undergoing prosthetic costal cartilage augmentation rhinoplasty from December 2020 to December 2021 were selected.According to the type of nasal base graft,135 cases (54.22%) of massive costal cartilage graft were divided into observation group and granule 114 cases (45.78%) in the costal cartilage graft group served as the control group.The operation time of the two groups was recorded,and regular follow-up was performed to measure the columella-lobular angle,columella-upper lip angle,nasal face angle,and nasal frontal angle before and after the operation.Compare the results of surgery and complications.Results The 249 patients were followed up for an average of 10.2±2.7 months.The operation time of the observation group was shorter (P<0.05).There was no statistical comparison between the two groups of preoperative columella-lobular angle,columella-upper lip angle,nasal face angle,and nasal frontal angle.Significance (P>0.05).The indexes of all angles after operation were improved compared with that before operation (P<0.05).After comparing the various angle indexes,the observation group’s increase of small column-lobular angle and small column-upper lip angle were better than those of the control group (P<0.05).The observation group’s satisfaction with surgery was significantly better than that of the control group (P<0.05).There was no significant difference in the incidence of incision infection and hematoma between the two groups (P>0.05).The incidence of graft displacement and unnatural upper lip in the observation group was significantly lower than that in the control group (P<0.05).The observation group had better overall facial coordination and nose softness.Conclusion Massive costal cartilage grafts have a significant effect on the correction of nasal alar base depression in prosthetic costal cartilage rhinoplasty.The curative effect and safety are reliable and it is worthy of promotion.
