The Clinical Effect of Sacubitril and Valsartan in the Treatment of Heart Failure With Decreased Ejection Fraction in the Elderly

Objective To focus on heart failure with decreased ejection fraction in the elderly(HFrEF) and use sacubitril and valsartan to evaluate the effect of its application and its influence on the incidence of adverse cardiovascular events. Methods Based on a specific time(January to December 2020) and a fixed range(in our hospital), elderly patients with HFrEF(90 cases) were selected and divided into 2 groups according to a random number table, and both were given conventional anti-heart failure programs(such as β-blockers, diuretics, etc.). Based on this, 45 patients in group A were treated with benazepril, and 45 patients in group B were treated with sacubitril and valsartan. Before and after treatment, two groups were compared in dimensional echocardiographic indicators, cardiac function and myocardial fibrosis indicators, and the occurrence of adverse cardiovascular events.Results After treatment, the left ventricular ejection fraction(LVEF) of group B was higher than that of group A, the difference was statistically significant(P < 0.05), and the left ventricular end diastolic volume(LVEDV) and left ventricular end systolic volume(LVESV) were lower than group A, the difference was statistically significant(P < 0.05). After treatment in group B, the soluble growth-stimulating expression gene 2 protein(sST2) and N-terminal B-type natriuretic peptide precursor(NTpro BNP) were lower than those in group A, the difference was statistically significant(P < 0.05), while the 6 min walk test(6 MWT) higher than group A, the difference was statistically significant(P < 0.05). The rate of readmission due to heart failure in group B(2.22%) was lower than that in group A(17.78%), the difference was statistically significant(P < 0.05).Conclusion For elderly patients with HFrEF, treatment with sacubitril and valsartan can improve their left ventricular function and left ventricular remodeling, and reduce the incidence of re-admission due to heart failure.
