Clinical effect of meropenem combined with sputum suction and bronchoalveolar lavage using fiber bronchoscope in the treatment of severe pulmonary infection in ICU

Objective To analyze the clinical effect of meropenem combined with sputum suction and bronchoalveolar lavage using fiber bronchoscope in the treatment of severe pulmonary infection in ICU.Methods A total of 76 patients with severe pulmonary infection treated in ICU of our hospital from April 2017 to April 2019 were selected as the research objects.According to different treatment methods,the patients were divided into control group(n=36) and study group(n =40).On the basis of routine treatment,the control group was treated with meropenem;the study group was treated with sputum suction and bronchoalveolar lavage using fiber bronchoscope on the basis of the control group.The clinical therapeutic effects of the two groups were compared.Results The fever regression time,pulmonary rale regression time,cough regression time,sputum negative conversion time and mechanical ventilation time in the study group were significantly shorter than those in the control group(P<0.05).After treatment,the CRP,IL-6,IL-10 levels and WBC in the study group were significantly improved,and those in the study group were better than the control group(P <0.05).After treatment,PaO₂and SaO₂in the two groups were significantly higher than those before treatment,PaCO₂was significantly lower than that before treatment,and those in the study group were better than the control group(P<0.05);after treatment,the MMV,FEV1 and FVC of the two groups were significantly higher than those before treatment,and those in the study group were higher than the control group(P<0.05).At 1 and 2 weeks after treatment,the CPIS score of the two groups was significantly lower than that before treatment,and that in the study group was lower than the control group,and the difference was statistically significant(P<0.05).Conclusion Meropenem combined with sputum suction and bronchoalveolar lavage using fiber bronchoscope in the treatment of severe pulmonary infection in ICU has a significant effect,it can improve the body’s inflammatory response,pulmonary function and arterial blood gas indexes,which is worthy of promotion.
