改良型临床肺部感染评分在呼吸机相关性肺炎预后评估中的价值

目的 探讨改良型临床肺部感染评分(CPIS)在呼吸机相关性肺炎预后评估中的价值。方法 选取2018年10月至2020年10月榆林市第二医院收治的100例呼吸机相关性肺炎患者作为研究对象。对所有患者进行呼吸机相关性肺炎的规范化治疗,并根据治疗后的效果将患者分为预后良好组(n=62)和预后不良组(n=38)。比较两组患者治疗前、后的血清学指标(PCT、CRP)、血气指标(PO₂、PCO₂、OI)及改良型CPIS评分,并分析改良型CPIS评分与PCT、CRP、PO₂、PCO₂、OI的相关性。结果 治疗前,两组的血清PCT、CRP水平比较,差异无统计学意义(P>0.05);治疗后,两组的血清PCT、CRP水平均较治疗前明显降低,且预后良好组低于预后不良组,差异具有统计学意义(P<0.05)。治疗前,两组的PO₂、PCO₂、OI比较,差异无统计学意义(P>0.05);治疗后,两组的PO₂、OI较治疗前明显升高,PCO_...

Value of modified clinical pulmonary infection score in prognostic evaluation of ventilator-associated pneumonia

Objective To explore the value of modified clinical pulmonary infection score(CPIS) in prognostic evaluation of ventilator-associated pneumonia.Methods One hundred patients with ventilator-associated pneumonia treated in Yulin No.2 hospital from October 2018 to October 2020 were selected as the research objects.All patients were treated with standardized treatment of ventilator-associated pneumonia.According to the effect after treatment,the patients were divided into good prognosis group(n=62) and poor prognosis group(n=38).The serum indexes(PCT,CRP),blood gas indexes(PO₂,PCO₂,OI) and modified CPIS score were compared between the two groups before and after treatment,and the correlation between modified CPIS score and PCT,CRP,PO₂,PCO₂,OI was analyzed.Results Before treatment,there were no significant differences in the levels of serum PCT and CRP between the two groups(P>0.05);after treatment,the levels of serum PCT and CRP in the two groups were significantly lower than those before treatment,those of the good prognosis group were lower than the poor prognosis group,and the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in PO₂,PCO₂ and OI between the two groups(P>0.05);after treatment,PO₂ and OI of the two groups were significantly higher than those before treatment,and PCO₂ was significantly lower than that before treatment,those of the good prognosis group were better than the poor prognosis group,and the differences were statistically significant(P<0.05).Before treatment,there was no significant difference in modified CPIS score between the two groups(P >0.05);after treatment,modified CPIS score in the two groups was significantly lower than that before treatment,that of the good prognosis group was lower than the poor prognosis group,and the difference was statistically significant(P<0.05).The Pearson correlation analysis results showed that the modified CPIS score of patients with ventilator-associated pneumonia was positively correlated with PCT,CRP and PCO₂,and negatively correlated with PO₂ and OI,and the differences were statistically significant(P <0.05).Conclusion Dynamic monitoring of the modified CPIS score of patients with ventilator-associated pneumonia can effectively evaluate the prognosis,and plays an important role in the process of diagnosis and treatment,with high clinical application value.
