头位脐带绕颈与分娩方式及新生儿窒息的关系

目的探讨头位脐带绕颈与分娩方式及新生儿窒息的关系。方法回顾性分析我院2009年足月单胎头位脐带绕颈产妇715例,并与同期815例无脐带绕颈者进行对照研究,记录、比较两组的分娩方式、胎儿窘迫发生率、新生儿窒息发生率。结果脐带绕颈组的剖宫产率、胎儿窘迫发生率均高于对照组,差异有统计学意义（P〈0.05）,新生儿窒息率脐带绕颈组（11.05%）较无脐带绕颈组（9.69%）高,但两组比较无统计学意义（P〉0.05）。结论脐带绕颈可使剖宫产率及胎儿窘迫发生率增加,但不增加新生儿窒息率。选择最佳的分娩方式和时机是降低新生儿窒息率的关键。

The Relationship of Head Position Umbilical Cord Around the Neck with Delivery Mode and Neonatal Asphyxia

Objective To explore the relationship of head position umbilical cord around the neck with delivery mode and neonatal asphyxia.Methods Seven hundred and fifteen cases of single month-enough head position umbilical cord around the neck maternals in our hospital in 2009 were retrospectively analyzed,and comparatively studied with 815 cases of maternals as control group who without umbilical cord around the neck hospitalized at the same periods,and recorded the delivery mode,the incidence of fetal distress and neonatal asphyxia of the two groups.Results Cesarean section rate,incidence of fetal distress of umbilical cord around the neck group were higher than those of the control group,the difference were statistically significant（P0.05）,the incidence of neonatal asphyxia of umbilical cord around the neck group（11.05%） was higher than that of without umbilical cord around the neck group（9.69%）,but the difference was not significant（P0.05）.Conclusion Umbilical cord around the neck can increase cesarean section rate and incidence of fetal distress,but does not increase the rate of neonatal asphyxia.Choose the best mode of delivery and timing are key to reducing the rate of neonatal asphyxia.
