激光血管成形术后血管的形态学变化

摘    要:用腹主动脉内膜球囊损伤造成狭窄≥50%的日本大耳白兔50只进行激光血管成形术(LA),术后3、7、14、21、28、35、42、49、56和63天血管取材切片作病理变化分析。结果:LA后14天,血管平滑肌细胞(SMC)出现增殖,动脉中层SMC向内层迁移;LA后28天,内膜层SMC增殖最为明显,并导致血管再狭窄;内膜层SMC表型与合成型SMC相似;LA后35天,内膜层增厚速度降低。结论:实验性LA后SMC增殖反应呈一动态过程,与球囊血管成形术后再狭窄的病理变化大致相同,故本实验模型适于LA后血管再狭窄机制的研究。

Abstract:Laser angioplasty (LA) was performed in the abdominal aorta of 50 rabbits after development of an atherosclerotic stenosis induced by balloon injury. The vessels were excised 3,7,14,21,28,35,42,49,56 and 63 days after laser treatment. The results showed that vascular smooth muscle cell (SMC) proliferation was found at 14 days and a restenosis of the aorta injured by laser occurred obviously due to intimal proliferation of SMC at 28 days after LA. Vascular SMC migration was observed from media to intima and the SMC were phenotypically very similar to synthesis type at 14 days after LA. After 35 days, increase of intima thickening slowed down. Conclusion: The proliferation response of SMC after experimental LA occurred as a dynamic process,resulting in a formation of restenosis within 4 weeks after LA. The extent of intimal hyperplasia in abdominal aorta after LA is comparable with the effect of transluminal balloon angioplasty in this experimental model,which can be applied to research of the mechanisms of restenosis induced by laser vascular injury.
