糖皮质激素对鼻息肉中水通道蛋白3表达的影响

摘    要:目的探讨水通道蛋白(AQP)与鼻息肉发病的关系,研究糖皮质激素控制治疗鼻息肉的作用机制。方法收集鼻息肉标本40例,其中术前局部使用鼻用糖皮质激素的鼻息肉标本20例,未使用糖皮质激素的鼻息肉标本20例,正常鼻腔下鼻甲黏膜标本7例。用免疫组织化学法检测水通道蛋白3的表达,并进行统计学分析。结果 AQP3在上皮细胞,腺体细胞,血管及血窦内皮细胞均有表达。上皮细胞中正常下鼻甲黏膜组AQP3阳性细胞数多于鼻息肉未用激素组,差异无统计学意义。腺体、血管及血窦内皮中正常下鼻甲黏膜组AQP3阳性细胞数少于鼻息肉未用激素组,差异有统计学意义。鼻息肉激素治疗组上皮细胞、腺体、血管及血窦内皮细胞中AQP3阳性细胞数少于鼻息肉未用激素组,差异有统计学意义。结论水通道蛋白3的正常表达与其维持下鼻甲功能有关,水通道蛋白3过度表达与鼻息肉的形成有关。局部使用糖皮质激素后水通道蛋白3在鼻息肉组织中的腺体、血管及血窦内皮细胞中表达下降,是糖皮质激素治疗鼻息肉的可能作用机制之一。

Abstract:Objective To study the relationship between aquaporin and nasal polyps and the mechanism of glucocorticoids on nasal polyps.Methods Twenty samples of nasal polyps which were given BUD intranasal spraying before operation,another twenty samples which were given no drug, and samples of normal were examined on the expression of AQP3 by immunohistochemical method and analysed with statistical method.Results AQP3 expressed in cells of epithelia,vascular endothelia and glandular epithelia.The difference of AQP3 positive cells in the mucosal epithelial was not significant between the normal group and the nasal polyps group.The expressions of AQP3 positive cells in the glandular epithelium and blood vessel endothelium were significantly less in the normal group compared with the nasal polyps group.The expression of AQP3 positive cells in mucosal epithelial,glandular epithelium and blood vessel endothelium was significantly less in the glucocorticoids group than in the nasal polyps group without using glucocorticoid.Conclusion AQP3 positive cells all express in mucosal epithelial,glandular epithelium and blood vessel endothelium of normal mucosa inferior turbinate,it may contribute to the function of inferior turbinate.The expression of AQP3 positive cells in glandular epithelium and blood vessel endothelium is more in nasal polyps,it may be a vital factor for the formation of nasal polyps.The expression of AQP3 positive cells in glandular epithelium and blood vessel endothelium is reduced in nasal polyps after using BUD intranasal spraying.It may be the explanation of why glucocorticoids can treat nasal polyps.
